FITNESS FOR DUTY MEDICAL EXAMS. ‌ In the event the Fire Chief or designee determines based on reasonable, objective evidence that an employee is experiencing problems in the performance of their duties and those problems are related to job performance or employee safety and are of a nature that they may be due to a medical condition, the Fire Chief or his/her designee may order that an employee undergo a medical examination to determine whether the employee is fit for duty. A physician selected by the District shall give the examination and the full cost of such medical examination shall be borne by the District, as shall any additional medical examinations required by this section. The employee shall be placed on administrative leave with pay at the same time the employee is ordered to undergo a medical examination and shall remain on administrative leave with pay until the District physician has issued a written determination of whether the employee is fit for duty. The District shall advise the employee in writing of the results of the medical examination immediately upon receipt of those results. In the event the employee disagrees with the results of the Medical Examination the employee may request a second opinion from a physician to be selected by mutual agreement between the employee and the District and the employee shall remain on administrative leave with pay until the District has received the second opinion and notified the employee of the results. The District and the employee shall work together in good faith to select the physician for the second opinion as soon as possible. In the event the second opinion is that the employee is fit for duty. The employee shall be reinstated to regular duty. In the event the second opinion is that the employee is unfit for duty, the employee shall be removed from administrative leave with pay, in which event the employee may then take such disability leave, Annual Leave, and/or other paid leaves of absence to which the employee is entitled under the terms and provisions set forth in ·this Memorandum of Understanding that are applicable in light of the employee's particular circumstances.   • Fitness for Duty Examination A. In directing employees to undergo a fitness for duty examination, the Agency will observe applicable rules and regulations.

  • Fitness for Duty When question exists related to appropriate leave administration or work safety to individuals, co-workers or others, the EMPLOYER may require employees to undergo a medical evaluation that will enable the EMPLOYER to determine the employee’s fitness for performance of his/her duties.   • Private Duty Nursing Services This plan covers private duty nursing services, received in your home when ordered by a physician, and performed by a certified home healthcare agency. This plan covers these services when the patient requires continuous skilled nursing observation and intervention.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. Preauthorization may be required for certain surgical services. Reconstructive Surgery for a Functional Deformity or Impairment This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia. Preauthorization may be required for these services.

  • Clinical Management for Behavioral Health Services (CMBHS) System The CMBHS is the official record of documentation by System Agency. Grantee shall:

  • Diagnostic Services Procedures ordered by a recognized Provider because of specific symptoms to diagnose a specific condition or disease. Some examples include, but are not limited to:

  • Hospice Care If you have a terminal illness and you agree with your physician not to continue with a curative treatment program, this plan covers hospice care services received in your home, in a skilled nursing facility, or in an inpatient facility.
