Life Insurance and AD&D. The Employer shall provide all eligible employees with Life Insurance and AD&D as per Appendix “A” of this Collective Agreement.
Life Insurance and AD&D. The Employer agrees to pay one-hundred percent (100%) of the premiums for a mutually agreed-upon life insurance and accidental death and dismemberment plan for participating employees.
Life Insurance and AD&D. The Hospital shall provide at no expense to 21 the nurse, a group life and accidental death and dismemberment (AD&D) insurance 22 policy for all benefit eligible nurses. Coverage will be equivalent to the nurse’s annual 23 base wage, as defined in the summary plan description, with a minimum of $35,000.
Life Insurance and AD&D. The Employer will continue a $17,000 life insurance and AD&D plan for each employee who participates in the benefit program. The Employer will pay one hundred percent (100%) of the cost of this plan.
Life Insurance and AD&D. On the employee’s 65th birthday, life insurance and AD&D benefit will terminate.
Life Insurance and AD&D a) The District will cover the full costs of Life Insurance and AD&D Coverage for the duration of this contract.
Life Insurance and AD&D. Life insurance, including that provided under Rowan’s Supplemental Life Insurance Plan, and AD&D, will cease as of your Separation Date. You will have a 31-day period thereafter to exercise the conversion options for life insurance according to the terms and conditions in effect at that time. It will be your responsibility to complete the conversion process if you so desire.
Life Insurance and AD&D. No change in current plan (one and one half times (1½X) annual pay including tips and tokes).
Life Insurance and AD&D. The Company agrees to pay the full cost of the premium for providing each employee with Group Life Insurance and Accidental Death and Dismemberment Insurance in the amount of $50,000 for each employee. This amount will increase to $51,000 effective year two (2) of this collective agreement. The Company agrees to offer the employees Optional Life Insurance in multiples of $10,000 up to a maximum of $60,000 and Optional Accidental Death and Dismemberment Insurance in multiples of $10,000 up to a maximum of $60,000. Following the completion of sixty (60) working days of continuous employment, employees may participate at any time for Optional Insurance Coverage. When applying for Optional Life coverage, an employee must submit satisfactory medical evidence of insurability to the insurer. Optional Life coverage will not become effective until the medical evidence of insurability is approved-in writing- by the insurance company. It is further understood that premiums for Optional insurance's will be deducted from the employee's pay cheque.
Life Insurance and AD&D. Subject to your payment of applicable premiums, during the Severance Period you will be able to participate in the life insurance program coverage and AD&D coverage generally available to all PennzEnergy Company salaried employees. You will have a thirty-one (31) day period after the end of the Severance Period to convert the balance of your employee life insurance, according to the terms and conditions in effect at that time. In addition, AD&D may be converted, according to the terms and conditions in effect at that time. Application must be made to the insurance company. It will be your responsibility to complete the conversion process if you so desire.
