HOSPICE PROGRAM. Hospice is a coordinated program of home and inpatient care that provides non-curative medical and support services for persons certified by a physician to be terminally ill with a life expectancy of six (6) months or less. Hospice programs provide patients and families with palliative and supportive care to meet the special needs arising out of physical, psychological, spiritual, social and economic stresses which are experienced during the final stages of illness and during dying and bereavement.

All services must be provided according to a written plan of care which reflects the changing needs of the patient/family. If an Enrollee in the Contractor’s plan becomes terminally ill and receives Hospice Program services he or she may remain enrolled and continue to access the Contractor’s Benefit Package while Hospice costs are paid for by Medicaid fee-for-service.
HOSPICE PROGRAM. Physician shall provide services as scheduled. Physician shall make routine visits and face to face visits for hospice at both patient homes and the Hospice House. Physician will document these visits and report any problems or issues to the attending physician on Monday following weekend coverage. Physician, functioning under his/her primary medical board scope of practice will take phone calls from the covered facilities and provide orders to the staff at the facility.
HOSPICE PROGRAM. Physician shall be paid on an hourly basis at a rate of Fifty and No/100 Dollars ($50.00) per hour Monday through Friday; and Fifty-Five and No/100 Dollars ($55.00) per hour Saturday and Sunday.
