LATE ENROLLEES. If You or Your Dependents do not enroll when first eligible, it will be necessary to wait for the next Open Enrollment Period. However, You may be eligible for special enrollment as set out below under the Special Enrollment Period section. OPEN ENROLLMENT The Open Enrollment Period is defined by the federal government. This enrollment period applies whether purchasing through the Health Insurance Marketplace or directly from Alliant Health Plans. SPECIAL ENROLLMENT PERIODS Consumers can qualify for a Special Enrollment Period (SEP) to enroll for health insurance coverage if they meet certain eligibility criteria. Below is a list of the categories of SEPs currently available. • Loss of qualifying health coverageChange in household size • Change in primary residence • Newly gaining eligibility for an Individual Coverage Health Reimbursement Arrangement (ICHRA) or Qualified Small Employee Health Reimbursement Arrangement (QSEHRA). • Enrollment or plan error. Loss of coverage does not include voluntary termination of coverage or other loss due to: • Failure to pay premiums on a timely basis, including COBRA premiums prior to expiration of COBRA coverage; or • Situations allowing for a rescission as specified in 45 CFR 147.128. WHEN YOUR COVERAGE BEGINS Coverage starts at 12:01 a.m. on the Effective Date. The Effective Date is determined by Alliant Health Plans. No benefits will be provided for services, supplies or charges incurred before Your Effective Date. CHANGING YOUR COVERAGE There will be an annual re-enrollment period during which time Members may elect to change their options. CHANGING YOUR COVERAGE (ADDING A DEPENDENT) You may add new Dependents to Your plan, during Open Enrollment or a Special Enrollment Period, by contacting Client Services at (000) 000-0000. If You purchased through the Health Insurance Marketplace, You must notify the Health Insurance Marketplace. If You purchased outside the Health Insurance Marketplace, You must notify Us in writing. Coverage is provided only for those Dependents You have reported to Alliant and added to Your coverage by completing the correct application during a time they are eligible to be enrolled. MARRIAGE AND STEPCHILDREN A Member may add a spouse and eligible stepchildren within thirty-one (31) days of the date of marriage. The Effective Date will be the date of marriage. LATE ENROLLEES. Unless otherwise prohibited by law, late enrollment is allowed only during the open enrollment period, which is the month prior to Your group’s next annual renewal date. Coverage for Late Enrollees will be effective on Your group’s next annual renewal date. Enrollment forms must be signed by the last day of open enrollment and must be received by Us in a timely manner. A Subscriber and his or her Eligible Dependents will not be considered Late Enrollees if the Group offers multiple health benefit plans and the Subscriber elects a different plan during the open enrollment period. Enrollment changes between these multiple plans are permitted only at open enrollment, unless for an HMO/POS enrollee who has moved out of the HMO/POS service area.

  • Community Based Adult Intensive Service (AIS) and Child and Family Intensive Treatment (CFIT) – AIS/CFIT programs offer services primarily based in the home and community for qualifying adults and children with moderate- to-severe mental health conditions. These programs consist at a minimum of ongoing emergency/crisis evaluations, psychiatric assessment, medication evaluation and management, case management, psychiatric nursing services, and individual, group, and family therapy. In a Provider’s Office/In Your Home This plan covers individual psychotherapy, group psychotherapy, and family therapy when rendered by: • Psychiatrists; • Licensed Clinical Psychologists; • Licensed Independent Clinical Social Workers; • Advance Practice Registered Nurses (Clinical Nurse Specialists/Nurse Practitioners- Behavioral Health); • Licensed Mental Health Counselors; and • Licensed Marriage and Family Therapists. Psychological Testing This plan covers psychological testing as a behavioral health benefit when rendered by: • neuropsychologists; • psychologists; or • pediatric neurodevelopmental specialists. This plan covers neuropsychological testing as described in the Tests, Labs and Imaging section.

  • TEACHING HOURS AND TEACHING LOAD A. As professionals, teachers are expected to devote to their assignments the time necessary to meet their responsibilities, but they shall not be required to “clock in or clock out” by hours and minutes. Teachers shall indicate their presence for duty by placing their signature and time in the proper column of the faculty “sign-in” roster.

  • CREDIT FOR PREVIOUS EXPERIENCE All employees shall be classified according to previous comparable supermarket experience. Previous comparable experience shall be granted on the following basis:

  • Dimensions Education Bachelor’s Degree in Computer Science, Information Systems, or other related field. Or equivalent work experience. Experience: A minimum of 4 years of IT work experience in data modeling, data analysis, relational DBMS design and support and relevant computing environments.

  • Teaching Assistants 1. All Teaching Assistants shall receive the same remuneration per base unit and the difference between the amount paid a Graduate Teaching Assistant and other Teaching Assistants is an associated scholarship. See XIII D. for appropriate compensation.

  • Teaching Staff Assigned to More Than One Building Each Educator who is assigned to more than one building will be evaluated by the appropriate administrator where the individual is assigned most of the time. The principal of each building in which the Educator serves must review and sign the evaluation, and may add written comments. In cases where there is no predominate assignment, the superintendent will determine who the primary evaluator will be.

  • Teaching Load Full teaching assignments shall normally include 12 course credit hours of scheduled teaching per academic quarter. A reassignment of duty, for the equivalent of 3 or 4 credit course, shall be provided during one term of the first academic year to all newly hired tenure track faculty to further their teaching, scholarship and service and to encourage faculty retention. Wherever possible the University will endeavor to arrange teaching schedules that avoid excessive numbers of preparations and recognize evening and/or off-campus assignments. Class sizes will be established and monitored by the appropriate academic xxxx in consultation with division chairs and affected faculty each term. The following equivalencies will be used in determining teaching assignments:

  • Pre-Observation Conference The pre-observation conference shall be held prior to each formal observation. The teacher and evaluator will mutually agree when to conference. The purpose of the pre-observation conference is to discuss the employee’s goals, establish a date for the formal observation, and to discuss such matters as the professional activities to be observed, their content, objectives, strategies, and possible observable evidence to meet the scoring criteria.

  • Post-Observation Conference 1. The post-observation conference between the evaluator and teacher will be held no later than five (5) days after the formal observation.

  • Study Population ‌ Infants who underwent creation of an enterostomy receiving postoperative care and awaiting enterostomy closure: to be assessed for eligibility: n = 201 to be assigned to the study: n = 106 to be analysed: n = 106 Duration of intervention per patient of the intervention group: 6 weeks between enterostomy creation and enterostomy closure Follow-up per patient: 3 months, 6 months and 12 months post enterostomy closure, following enterostomy closure (12-month follow-up only applicable for patients that are recruited early enough to complete this follow-up within the 48 month of overall study duration).
