Life Insurance and Accidental Death & Dismemberment. The Board shall provide a MESSA life insurance benefit in the amount of $20,000 with accidental death & dismemberment coverage.
Life Insurance and Accidental Death & Dismemberment. Effective January 1, 2010, the City shall be responsible for paying premiums for a life insurance and Accidental Death and Dismemberment (AD&D) policy for each employee. The life and AD&D policy shall provide a $5,000 life insurance and a $5,000 AD&D benefit.
Life Insurance and Accidental Death & Dismemberment. Full-time employees shall participate in a group plan of Life and Accidental Death and Dismemberment Insurance with the Corporation paying 100% of the premiums. Coverage for employees shall be equal to one and one-half (1.5) times the annual basic salary or wages rounded to the next highest $500 up to age seventy (70) or as prescribed by the carrier. Full-time employees age seventy (70) or greater are subject to life insurance volume reductions as prescribed by the insurance carrier.
Life Insurance and Accidental Death & Dismemberment. The City pays premiums for a life insurance and Accidental Death and Dismemberment (AD&D) policy for each employee. The life and AD&D policy shall provide a $5,000 life insurance and a $5,000 AD&D benefit. Members of the Firefighters Association may choose to subscribe to a long-term disability (LTD) plan other than that offered by the City as long as there is no cost to the City.

The amount of insurance reduces by 50% at age 65 and terminates at age 70, retirement, or termination of employment.

The Company shall provide, at no cost to the employee, term life insurance in the amount of two (2) times their annual salary up to a maximum of five-hundred thousand ($500,000) and Accidental Death & Dismemberment coverage in the amount of two (2) times their annual salary up to a maximum of one-hundred thousand ($100,000). The employee may elect to purchase supplemental life coverage (up to 4X their annual salary or the maximum allowable by the plan) under this plan for himself/herself and/or to include his/her lawful dependants; the employee will be responsible for the additional premium costs for himself/herself and/or 100% of the total premium costs for his/her lawful dependants. All contributions will be collected through normal payroll deductions.
Life Insurance and Accidental Death & Dismemberment. The School provides employees who are regularly scheduled to work a minimum of thirty (30) hours per week with life insurance at no cost to the employee. The amount of insurance equals an employee’s annual salary up to a maximum limit of $50,000.

(a) The Employer shall contribute of the billed premium toward coverage of full-time eligible nurses in the group life insurance plan which provides coverage at two (2) times annual salary. Such insurance shall include benefits for Accidental Death and Dismemberment, up to two (2) times annual salary. Eligible employees may apply for optional life insurance in accordance with the requirements of the benefitplan. This life insurance is payable by the employee. Eligible employees may apply for Spousal Life Insurance in accordance with the requirements of the benefit plan. This life insurance is payable by the employee. The Employer shall continue to pay, for a period of up to two (2)years, its portion of the premiumfor benefitplans for eligible nurses who are on paid leave of absence or Nurses who are on layoff may continue to participate in benefit plans, at their request, providedthey make arrangements for payment of the full premium cost and provided that the layoff does not exceed one (1) year. Retired nurses may continue to participate in the benefit plans which permit participation by a non salaried nurse. The nurses shall pay the cost of the premiums. The Employer may substitute another carrier for any of the foregoing plans (other than provided that the level of benefits conferred thereby are not decreased. The Employer will advise the Association of any change in carrier or underwriter at least sixty (60) days prior to implementing a change in carrier. The Employer shall provide each nurse with information booklets outlining the current provisions in the insured benefit plans set out above. Upon request, the Employer will make the plans available to the Association. The benefit plans are those described in policy as amended by this collective agreement.
