Optional Accidental Death and Dismemberment Insurance. This plan is available on an optional, employee-pay-all basis and enables you to purchase additional amounts of accidental death and dismemberment insurance on an employee and/or family plan basis. Coverage is available from a minimum of $10,000 to a maximum of $300,000 in $10,000 increments.

Optional Accidental Death and Dismemberment Insurance. Optional Accidental Death and Dismemberment Insurance in units of $10,000.00 to a maximum of $200,000.00 will be made available to members of the Bargaining Unit in the Group Life Insurance plan. Such coverage will be at the member’s expense and at the group rate. Amounts and insurability will be at the sole discretion of the insurance carrier.

Critical Conditions Insurance will provide a lump sum payment to insured employees in the event he/she and/or dependents are afflicted, while coverage is in force, with a critical condition as defined in the policy.

Optional Accidental Death and Dismemberment Insurance. Leave Plan. The Employer will provide a leave Plan for the two (2) week waiting period under the Employment Insurance Act of of the employee's salary. Commencing with the third week of the leave, employees granted leave shall be entitled to receive a fifteen (15) week benefit of per week. No payment shall be made for any periods during which the employee is not otherwise entitled to earn pay. Effective July the Employer shall contribute of the required premiums for coverage described in (a), and above. Employees shall pay of premium costs for coverage described in and (e) above.
