LONG TERM DISABILITY ELIGIBILITY. Employees covered under the LTD plan become eligible to receive LTD benefits following absence from work for six (6) continuous months due to illness or injury. It is understood that during the eligibility period if an employee returns to work and absents himself/herself within thirty (30) days of the return date due to the same disability or a related cause, there is no requirement to serve an additional six month eligibility period. However, the initial six (6) month eligibility period will be extended by the number of days the employee returned to work.

LONG TERM DISABILITY ELIGIBILITY. LTD coverage will apply to full-time personnel. Coverage will be extended to personnel employed thirty hours a week or more, with the District paying only that portion of the premium corresponding with the percent of full-time employment as reflected in the work schedule and contract of the employee. Employees working less than full-time but more than thirty hours a week will pay the balance of the premium, through payroll deductions.

ENTITLEMENT TO OTHER BENEFITS WHILE AWAITING An employee who is eligible to receive Long Term Disability benefits, who has completed probationary period and who is on extended illness or injury and who uses all accumulated sick leave prior to the commencement of long term disability, will continue to be covered, in accordance with the terms of the applicable insurance policy, for the following benefits: Dental Plan Paid by Employer Extended Health Care Paid by Employer (including drugs and vision care) Employer's Health Tax Paid by Employer Life Insurance Paid by Employer Long Term Disability Paid by Employer Waiver of employee contributions on the first day of the fifth month of illness or injury Employees receiving the above benefits pro-rata are entitled to have those benefits maintained so long as the employee contributions are maintained.
LONG TERM DISABILITY ELIGIBILITY. Employees may purchase Long Term Disability (LTD) coverage on a voluntary basis through payroll deduction. Eligibility will be the 1st of month following date of hire. Temporary employees are not eligible for this benefit. Payroll deduction will be taken on a post-tax basis the first and second pay periods of each month.

  • Long-Term Disability (income replacement during a qualifying disability equal to sixty-six and two-thirds percent (66 2/3%) of basic monthly earnings to the established maximum following a one hundred and twenty (120) working day elimination period);

  • Short-Term Disability The Employer agrees to provide Short Term Disability benefits to all active full-time employees from the first (1st) day of an accident or the first (1st) full-time day of hospitalization or the fourth (4th) day of sickness. The Plan will pay sixty-six and two thirds percent (66 2/3%) of basic earnings for the first two (2) weeks, then Unemployment Insurance will pay fifteen (15) weeks, then the Plan will resume payments for thirty-five (35 weeks).

  • Disability If the Executive's employment is terminated by reason of the Executive's Disability during the Employment Period, this Agreement shall terminate without further obligations to the Executive, other than for payment of Accrued Obligations and the timely payment or provision of Other Benefits. Accrued Obligations shall be paid to the Executive in a lump sum in cash within 30 days of the Date of Termination. With respect to the provision of Other Benefits, the term Other Benefits as utilized in this Section 6(c) shall include, and the Executive shall be entitled after the Disability Effective Date to receive, disability and other benefits at least equal to the most favorable of those generally provided by the Company and its affiliated companies to disabled executives and/or their families in accordance with such plans, programs, practices and policies relating to disability, if any, as in effect generally with respect to other peer executives and their families at any time during the 120-day period immediately preceding the Effective Date or, if more favorable to the Executive and/or the Executive's family, as in effect at any time thereafter generally with respect to other peer executives of the Company and its affiliated companies and their families.

  • Long-Term Disability Insurance 250. The City, at its own cost, shall provide to employees a Long Term Disability (LTD) benefit that provides, after a one hundred and eighty (180) day elimination period, sixty percent salary (60%) (subject to integration) up to age sixty-five (65). Employees who are receiving or who are eligible to receive LTD shall be eligible to participate in the City's Catastrophic Illness Program as set forth in the ordinance governing such program.

  • Total Disability (a) Total disability, as used in this Plan, means the complete inability because of an accident or sickness of a covered employee to perform all the duties of his/her own occupation for the first two (2) years of disability. Thereafter, employees able by reason of education, training or experience to perform the duties of a gainful occupation for which the rate of pay is not less than seventy-five percent (75%) of the current rate of pay of their regular occupation at date of disability will not be considered totally disabled and will therefore not be eligible for benefits under this Long Term Disability Plan.
