Medical Note. (a) An employee absent for more than five (5) consecutive working days may be asked to furnish within seven (7) working days from commencement of absence, a medical note from her physician certifying the duration of illness. Medical Note. At any time, a medical note may be requested by the Employer to substantiate an employee’s absence subject to section 50 subsection (13) of the Employment Standards Act, 2000 and/or an employee’s fitness to perform the duties/responsibilities of the position. The cost of such medical note shall be borne by the Employer.
Medical Note. 27.2.1 Employees returning to work after more than five (5) consecutive days of sick leave for a non-serious illness shall submit a medical note from a health care provider to their supervisor or to HR. For serious illness, see Section 27.17
Medical Note. The Employer reserves the right to request medical documentation from an employee who has been absent for more than three (3) consecutive shifts, when they have a pattern of absences, excessive absenteeism, when the Employer has serious doubt as to the validity of a medical leave of absence.
Medical Note. Any employee who will not be at work on their scheduled shift due to illness or other reasons, must call and leave a message on the Employer’s designated phone, prior to the start of the shift. For absences of three (3) or more days due to illness, the Employer may request the employee to provide a doctor’s note. In such cases and the Employer requests for accommodation information, the Employer will pay for the cost of the doctor’s note, statement or forms up to a maximum of $80.00. The Employer will not pay for medical notes or forms related to insurance claims or forms, or other employee requests.
Medical Note. After an Employee has been absent on sick leave for ten (10) successive working days, a written statement from the Employee's medical practitioner specifying the general nature and prognosis of the illness may be required.
