Medical Notes. Where there is a cost to the employee for additional medical or other information requested by the Company or the Insurer of the Company, the Company shall reimburse the employee to whom the request was made.
Medical Notes. An employee who is required to attend a medical examination shall not be required to do so on his/her assigned rest days. When such an examination takes place during normal working hours, the employee shall be compensated for time lost. When the Company requires a note from the doctor, the Company will pay for doctor note(s) up to a maximum of one hundred ($100) per calendar year. An employee, who is scheduled to work and is unavailable due to sickness, may be required to provide a doctor’s note upon request after the third (3rd) day. The Company may, in its discretion, request a medical certificate after one (1) day in reasonable circumstances.
Medical Notes. During current negotiations the Union raised concerns regarding supervision requesting employees to supply doctor/nurse practitioner statements in cases of continuing absenteeism. The Company stated that in cases of this nature, supervision are expected to use discretion and good judgement in making these requests of an employee. This method of controlling absenteeism should be used only when supervision has reason to believe that the employee's absence was without just cause.
Medical Notes. Upon providing a paid receipt employees will be reimbursed for up to $25 for medical notes, and reasonable costs for specialists’ notes and reports, required by this Agreement or requested by the Corporation.
Medical Notes. In the event the Employer requires or requests an employee to provide a medical note, the fee charged by a medical practitioner for completing such medical note will be paid for in full by the Employer. The employee must submit their receipt for the medical note which identifies the fee charged and paid by the employee. The Employer will reimburse the employee in the pay period immediately following the employee submitting such receipt. The Employer shall not request medical notes unless there is a reasonable basis to do so.
Medical Notes. .1 The company may, in certain circumstances ask for a medical note as evidence to substantiate an absence.
.2 When a medical note is required is must contain the following:
.2.1 the name of the physician or medical practitioner who is providing the note;
.2.2 the date on which the employee was seen
2.3 a clear indication of the employee’s functional abilities and medical restrictions; and
Medical Notes. During negotiations, the Company and the Union had comprehensive discussions regarding medical notes for Emergency Day Leaves as defined under the Employment Standards Act.2000 including the requirement of medical notes when Emergency Day Leaves have been exhausted. It is the Company’s intent to require an employee to provide medical notes as follows:
Medical Notes. Employees will be reimbursed up to $10.00 for each paid receipt for a medical note required by this Agreement or requested by the Corporation.
Medical Notes. In the event that the Company requests a medical note or report, upon receiving the note or report and a receipt from the Doctor, the Company shall reimburse the employee for the cost of the medical note or report.
