Performance Progress Reporting. SUB-RECIPIENT shall provide progress reporting to MARC using a ‘Brownfields Assessment Program Progress Report’ template to be provided. Reports will include current program status on each xxxxxxxxxx site, project type, subcontract status, project cost and program disbursements including grant administration to-date and brief project narrative. Reports are to be submitted according to the following schedule. Incomplete reports will be returned. a. Quarter ending 9/30/2023, progress reporting shall be submitted on or before October 10, 2023; b. Quarter ending 12/31/2023, progress reporting shall be submitted on or before January 10, 2024 along with a plan for encumbering remaining funds; c. Quarter ending 3/31/2024, progress reporting shall be submitted on or before April 10, 2024; d. Quarterly reporting must continue until all funds have been disbursed and expended and a final report shall be submitted no later than June, 2024 2025 or 45 days following the date all of the funds have been expended, whichever comes first.

Performance Progress Reporting. The Annual Performance Progress Report is due no later than 30 days prior to the end of the budget period (e.g. May 31, 2020), and serves as the continuation application for the follow-on budget period. All publications and manuscripts published as a result of the work supported in part or whole by this cooperative grant must be submitted with the performance progress reports. Additionally, health advisories, health education materials, and other products should be submitted. Annual Performance Progress Report should be submitted to XXXXXXX@xxxx.xx.xxx. Progress reports should include measures proposed by the Grantee, depending on the activities selected for the work plan. In addition, all grantees shall report the following on an annual basis: • Number and classification of staff hired under this contract • Data on incident/outbreak response: o Number of infectious disease control activities that were identified, by type (e.g., respiratory, food-borne, etc) o Number of infectious disease control activities by type o Number of infectious disease control activities supported by resources funded by this funding stream An infectious disease control activity is defined as one of the following: • An outbreak that meets a standard local health department, CDPH or CDC definition. • A response to an outbreak that exists in another jurisdiction, but has not yet impacted the responding jurisdiction. For example: implementing expanded hepatitis A vaccination to prevent an outbreak from spreading to the responding jurisdiction. • An investigation of a significant disease exposure with the potential to become an outbreak. For example: contact investigation of a large scale exposure to a single case of tuberculosis or measles (e.g., in a school). • A response to an infectious disease threat with the potential to become an outbreak, including a cluster of cases. For example, monitoring persons exposed to Ebola virus or investigating a cluster of cases of food-borne illness identified by Pulsed-Field Gel Electrophoresis or Whole Genome Sequencing.
Performance Progress Reporting. The Work Sponsor agrees to evaluate each participant's performance once a month during the Work Experience activity, and provide a comprehensive evaluation at the end of each participant’s term.

  • Performance Review Where a performance review of an employee’s performance is carried out, the employee shall be given sufficient opportunity after the interview to read and review the performance review. Provision shall be made on the performance review form for an employee to sign it. The form shall provide for the employee’s signature in two (2) places, one (1) indicating that the employee has read and accepts the performance review, and the other indicating that the employee disagrees with the performance review. The employee shall sign in only one (1) of the places provided. No employee may initiate a grievance regarding the contents of a performance review unless the signature indicates disagreement. An employee shall, upon request, receive a copy of this performance review at the time of signing. An employee’s performance review shall not be changed after an employee has signed it, without the knowledge of the employee, and any such changes shall be subject to the grievance procedure of this Agreement. The employee may respond, in writing, to the performance review. Such response will be attached to the performance review.

  • Progress Reports The Recipient shall submit to the OPWC, at the OPWC's request, summary reports detailing the progress of the Project pursuant to this Agreement and any additional reports containing such information as the OPWC may reasonably require.

  • Annual Performance Review The Employee’s performance of his duties under this Agreement shall be reviewed by the Board of Directors or a committee of the Board of Directors at least annually and finalized within thirty (30) days of the receipt of the annual audited financial statements. The Board of Directors or a committee of the Board of Directors shall additionally review the base salary, bonus and benefits provided to the Employee under this Agreement and may, in their discretion, adjust the same, as outlined in Addendum B of this Agreement, provided, however, that Employee’s annual base salary shall not be less than the base salary set forth in Section 4(A) hereof.

  • Performance Reviews The Employee will be provided with a written performance appraisal at least once per year and said appraisal will be reviewed at which time all aspects of the assessment can be fully discussed.
