Performance Requirements of HIA. 2.8.1 HIA shall provide infusion therapy services, including but not limited to intravenous, subcutaneous, intrathecal, epidural infusion therapies. Such agencies shall also provide specialty pharmacy services as well as home health nursing. Ambulatory infusion center services and/or provision of other associated enteral products and supplies specific to each patients’ needs will be preferred services. 2.8.2 HIA shall provide all associated supplies, equipment, medication and infusion solutions and nursing services in accordance the hospice services to be provided by HIA specific to each patient, including any required subcontracting coordination. To the extent that any subcontracting may be necessary, HIA shall ensure that the same terms and conditions that it may be bound by in this Agreement shall apply to its subcontractors. HIA will perform all pharmacy compounding using aseptic techniques in a laminar flow hood and provide all other in home skilled care and supervision necessary to provide home infusion therapy services and other nursing services in conjunction with home infusion if ordered by physician to eligible patients’ of UCI. 2.8.3 Patient eligibility shall be determined by the referring UCI Medical center and accepting HIA and referrals shall be generated by UCI Medical Center attending physician and its care team. All infusion therapy, nursing services, and other in home skilled care shall be provided by the HIA, but under the specific orders of the UCI Medical Center attending physician and/or its primary care team. The specific order shall outline the level and duration of service required for an eligible patient and any extension of services beyond the indicated duration, changes in medication, or any interim services, shall require approval of the attending physician or primary‐care physician. When a patient is discharged from the UCI hospital, the attending physician or primary care shall be the contact person for any changes to the original order. 2.8.4 After initial physician's order and team assessment, monitoring and adjustment of antibiotic, TPN therapy and other skilled in home services will be the responsibility of the home infusion agency. This will include the ordering and assessment of all necessary labs. HIA shall provide an infusion case coordinator who will coordinate all information needed by home care nursing and pharmacy for each patient to receive home infusion services and other related in home skilled services as orde...

  • Performance Requirements A. There is no guaranteed minimum amount of work which will be ordered under this Contract. B. The total Contract amount will not exceed $4,900,000. C. This is a Contract for work specified in individual Job Orders. Work ordered prior to but not completed by the expiration of the Contract period, and any additional work required as a result of unforeseen conditions encountered during construction up to six (6) months after the contract expiration date, will be completed with all provisions of this Contract still in force. Performance time for each Job Order issued under this Contract will be determined in accordance with the Contract. This performance time will be determined and agreed upon by both Parties for each individual Job Order. Contractor must self-perform 20% of the Work under this Contract for ‘A’ and ‘B’ licenses.
