Record Keeping System. Provider shall maintain an adequate record system for recording services, service providers, charges, dates and all other commonly accepted information elements for Covered Services rendered to Members under the terms of this Agreement (including but not limited to such records as are necessary for the evaluation of the quality, appropriateness, and timeliness of services performed). Company, Members and their representatives shall be given access to and can request copies of Members’ Medical Records to the extent and in the manner provided by La. R.S. 40:1299.96 and 45 C.F.R. § 164.524 and as subject to reasonable charges.
Record Keeping System. If Provider is a Participating Provider, Subcontractor shall, based on written policies and procedures, develop and maintain a record keeping system that: (a) includes sufficient detail and clarity to permit internal and external review to validate encounter submissions and to assure Medically Appropriate services are provided consistent with the documented needs of the Member; (b) conforms to accepted professional practice; and (c) allows the Subcontractor to ensure that data submitted to Contractor is accurate and complete by: (i) verifying the accuracy and timeliness of reported data; (ii) screening the data for completeness, logic, and consistency; and (iii) collecting service information in standardized formats to the extent feasible and appropriate. Record Keeping System. MDS shall establish and maintain, at its own expense, bookkeeping, accounting, and record keeping systems, including the preparation and retention of books and records. ESPRE shall have the right to review a statement summarizing Gross Revenues receipts on an annual basis upon ten (10) Business Days’ prior written notice to MDS. MDS shall provide a report to ESPRE of all persons or entities from which Gross Revenues have been derived in the previous quarter. ARTICLE IV.
Record Keeping System. A. By August 1, 2024, the District will institute a record-keeping system to identify the number of students with disabilities who participate in the High School’s Advanced Placement (AP) course offerings, including documenting the number of students with disabilities in each AP course beginning with the 2024-2025 school year. The system will also identify the overall number of students with disabilities enrolled in the District, and the number and percent of students with disabilities enrolled in each individual AP course.
B. By August 1, 2024, the District will institute a record-keeping system to ensure consistency and accuracy in the data reported to OCR’s Civil Rights Data Collection (CRDC).
Record Keeping System. CONTRACTOR shall maintain a system of record keeping that will allow CONTRACTOR to determine when it has incurred seventy-five percent of the total contract authorization under this Contract. Upon occurrence of this event, CONTRACTOR shall send written notification to DPSS at the address herein provided in Exhibit D - County’s Administration.

Provider shall maintain a record keeping system that meets all of the following standards: (i) provides sufficient detail and clarity to permit internal and external quality review to validate encounter data submissions and to assure Enrollees have been, and are being, provided with Medically Necessary Covered Services consistent with the documented needs of the Enrollee; (ii) conforms to accepted professional practices and all Applicable Laws related thereto; and (iii) is supported by written policies and procedures, which will allow Payers to verify the accuracy and timeliness of the data and screen for completeness, logic and consistency. Provider further agrees to collect information in standardized formats to the extent feasible.
Record Keeping System. The District will develop a record-keeping system of interpreter and translator services requested related to the newcomer program. The system will include information on the interpreter and translator services requested, including the primary language of the student/parents and their names, the date of the request and the date services were provided, the source and name of any interpreter or translator used (e.g., bilingual staff, staff interpreters or translators, contract interpreters/translators, community volunteers, telephone interpreter lines), and the reason for/purpose of the request. The District will develop a procedure to track the system to ensure that communication needs of the national-origin language-minority parents are being effectively met through appropriate use of interpreters and translators.
Record Keeping System. If Subcontractor is a Participating Provider, Subcontractor shall, based on written policies and procedures, develop and maintain a record keeping system that: (a) includes sufficient detail and clarity to permit internal and external review to validate encounter submissions and to assure Dentally Appropriate services are provided consistent with the documented needs of the Member; (b) conforms to accepted professional practice; and (c) allows the Subcontractor to ensure that data submitted to Contractor is accurate and complete by: (i) verifying the accuracy and timeliness of reported data; (ii) screening the data for completeness, logic, and consistency; and (iii) collecting service information in standardized formats to the extent feasible and appropriate.
36.1 Subcontractor shall provide data used for analysis of delivery system capacity, consumer satisfaction, financial solvency, encounters, utilization, quality improvement, and other reporting requirements under the Agreement to Contractor sufficiently in advance to allow Contractor to reasonably meet its reporting obligations under the OHP Contract.
Record Keeping System. Describe data to be collected and provide a sample monitoring form designated to track data on pest location, populations, harborage, trends in populations, status of previously suggested pest exclusion and prevention measures for which facility staff are responsible, and other relevant information. Department of Defense (DD) form 1532-1 or a smiliar form should be used for record keeping. NOTE: The invoices shall be itemized with the names and amount for each facility.
