Report Submittal Requirements. 2100 Contractor shall submit monthly reports within fourteen (14) calendar days after the end of the calendar 2101 month and annual reports no later than forty-five (45) calendar days after the end of each calendar year. 2102 Monthly and annual reports shall include at a minimum, all data and information described in Exhibit D, 2103 unless otherwise specified under this Agreement. 2104 Contractor may propose report formats that are responsive to the objectives and audiences for each 2105 report. The format of each report shall be approved by the City Franchise Contract Administrator, in their 2106 sole discretion. City Franchise Contract Administrator may, from time to time during the Term, review and 2107 request changes to Contractor’s report formats and content, which includes but is not limited to, any 2108 information that the City must report to a local or State agency, and Contractor shall not unreasonably 2109 deny such requests. 2110 Contractor shall submit all reports to the City Franchise Contract Administrator electronically via e-mail 2111 using software acceptable to the City. The City reserves the right to require the Contractor to maintain 2112 records and submit the reports required herein through use of a City-selected web-based software 2113 platform, at the Contractor’s expense. 2114 City reserves the right to require Contractor to provide additional reports or documents as City Franchise 2115 Contract Administrator reasonably determines to be required for the administration of this Agreement or 2116 in compliance with Applicable Law. 2117 6.3 Performance Review and Audit 2118 The City may conduct, and Contractor shall cooperate with, two (2) performance reviews and detailed 2119 financial audits, at any point during the Term of this Agreement in the City Franchise Contract 2120 Administrator’s sole discretion, to verify Contractor has fulfilled its financial and operational obligations 2121 under this Agreement. The purpose of such review and audit shall be, without limitation, to review 2122 Complaints, xxxxxxxx, and City Reimbursements, and to determine if Contractor has met the performance 2123 standards described in this Agreement (including, without limitation, direct services provided to 2124 Customers as described in Exhibit B, public education and outreach required in Exhibit C, recordkeeping 2125 and reporting as required in Exhibit D, and performance standards established in Exhibit F).   • EDD Independent Subrecipient Reporting Requirements Effective January 1, 2001, the County of Orange is required to file in accordance with subdivision (a) of Section 6041A of the Internal Revenue Code for services received from a “service provider” to whom the County pays $600 or more or with whom the County enters into a contract for $600 or more within a single calendar year. The purpose of this reporting requirement is to increase child support collection by helping to locate parents who are delinquent in their child support obligations. The term “service provider” is defined in California Unemployment Insurance Code Section 1088.8, Subparagraph B.2 as “an individual who is not an employee of the service recipient for California purposes and who received compensation or executes a contract for services performed for that service recipient within or without the State.” The term is further defined by the California Employment Development Department to refer specifically to independent Subrecipients. An independent Subrecipient is defined as “an individual who is not an employee of the ... government entity for California purposes and who receives compensation or executes a contract for services performed for that ... government entity either in or outside of California.” The reporting requirement does not apply to corporations, general partnerships, limited liability partnerships, and limited liability companies.
