Request for Credit. 6.1 Customer must request any credit in writing to Allstream’s Customer Care Representative, making reference to the trouble ticket, within thirty (30) days of the Service Outage. For calculating credit allowances, every month is considered to have thirty (30) days.
6.2 The credits outlined above shall not be compounding. For any particular Service Outage, Customer may not be eligible for more than one (1) credit with respect to any Service Outage. Customer’s total service credit(s) in any one (1) month will not exceed one (1) month’s MRC for the affected Service and do not apply to MRCs of other Services. If Customer fails to submit its request to Allstream in the manner set forth herein within, Customer will have waived its right to such service credits for that month. To be eligible for service credits, the Customer must be in good standing with Allstream and current in all of its obligations.
6.3 Service credit(s) may be granted only if Customer has afforded Allstream reasonable access to Customer’s premises for appropriate repairs, maintenance, testing, and any other work in order to remedy the cause of the Service Outage.
6.4 Allstream’s records and data will be the sole basis for all Service credit calculations and determinations.
Request for Credit. The Company, on or after the date of ------------------ this Agreement, shall give the Issuing Bank notice of its request for the issuance of a Letter of Credit by delivering to the Issuing Bank (with a copy to the Agent) a duly executed and completed L/C Application on Issuing Bank's then current form (herein, an "L/C Application"). Such request shall specify: (i) the date on which the issuance of the Letter of Credit is requested to be made (which day shall be a Banking Day), and (ii) the amount of the Letter of Credit. Subject to the conditions herein, the Issuing Bank will issue the Letter of Credit as soon as reasonably practicable after receiving the above described notice.
Request for Credit. Customer may request a credit for any Reporting Period under this SLA and such requests must be submitted in writing within ten (10) days after the end of the Reporting Period. The total of all credits applicable to or accruing in any given Reporting Period shall not exceed 50% of the Fees attributable to Kentik by Customer for the Reporting Period. All credit requests will be verified against Kentik system records. Xxxxxx will make available to Customer a monthly record upon request. Generally, credits will be applied to future invoices, but may also be received as refunds upon request by Customer.
Request for Credit. Each request for an advance under the Line of Credit will be made by a disbursement request in a form acceptable to Bank executed by an Authorized Officer, or by any other means acceptable to Bank.

Such Request for Letter of Credit shall specify: (i) the date on which the issuance of Letter of Credit is requested to be made (which day shall be a Business Day); and (ii) the amount of the Letter of Credit.

For each disbursement or extension of credit under this Agreement, Bank shall have received a complete and accurate Request for Credit from Borrower as described below, and Bank shall have determined that all conditions contained in this Agreement to the disbursement set forth in the Request for Credit have been met.

Any request for credit under Section 3.6.2(i)-(iv) must be made within fifteen (15) days of performance failure following the month in which the failure occurred, and shall include date time and duration of the interruption as well as a specific list of Critical Business Functions that were unavailable. In instances of uncertainty or dispute as to whether an actual service interruption has occurred, Consultant shall make a final determination with respect to the uncertainty and/or dispute, in good faith, based on its review of data provided by system logs, monitoring reports and configuration records, which Consultant shall make available to City, at City’s request. The Consultant shall provide such data to dispute any credit requests within 15 days of City’s submittal of such request.
