Separate periods of disability. Two or more separate periods of disability due to the same or related causes, which are separated by less than four months of unrestricted regular duty, will be deemed to be one period of disability. Such periods will be added together to determine waiting periods, if any, and to determine maximum payment and benefit periods.

Separate periods of disability. 49 A period of disability ends and benefit payments under this plan stop when you no longer are disabled or 50 you return to work for 1 full day. If you incur a second period of disability, the cause of the second 51 disability and the length of your recovery time between the disability periods will determine whether the 52 second disability is treated as a temporary recovery (that is, a continuation of the first disability claim) or 53 as a separate disability claim. 54 55 Your recovery will be considered a temporary recovery if, during the benefit payment period, you cease to 56 be disabled for a total of 60 days or less. The following provisions apply to periods of temporary recovery: 1 • Only 1 benefit waiting period applies. 3 • Your weekly salary used to determine your initial short-term disability benefit does not change. 5 • No short-term disability benefits are paid for the period of temporary recovery. 7 Your second period of disability will be considered a separate disability claim if you have returned to work 9 for 1 full day and 10 • It is due to a different cause than the first disability period, or 12 • It is due to the same cause or causes but your recovery is longer than 60 days, or 14 • The first period of disability began before you were covered under this plan. 16 You must submit a claim for benefits and meet the waiting period requirements before benefits will be paid. 18 Other Income Benefits 20 Certain other income benefits that you may be entitled to receive will reduce your weekly benefit from the 21 Short-Term Disability Plan. There is no minimum benefit payment under this plan. You must apply for 22 all other income benefits for which you may be eligible, including Social Security benefits (but excluding 23 retirement benefits). 24 25 Your benefits under this plan are reduced by the following sources of income: 26 • Salary continuation (to the extent combined short-term disability, salary continuation, and other 28 income benefits exceed 100% of predisability earnings). 29 • Benefits from insured or uninsured disability income plans of any employer, multiemployer or 31 multiple-employer welfare plan, or union welfare plan. 32 • Benefits from a disability income plan of any state or other jurisdiction. 34 • Social Security disability or retirement benefits, including primary, spouse, and dependent child 36 benefits. 37 38 • Railroad Retirement Act benefits, or other benefits paid under a Federal or state law. 39 40 • Workers’ comp...
