Supervisor's File. ‌ Nothing in this Article precludes an employee's Supervisor from maintaining a file and/or dated anecdotal notes regarding an employee. However, such notes are not considered part of the employee's official personnel file and will only be considered for action purposes when documented and placed in the official file.

Supervisor's File. The purpose of the supervisor file is to track work in progress, performance related concerns, successes, and similar information, to be used as a reference for up-coming annual employee evaluations. Each year, documentation in the supervisor’s file will be removed and destroyed after the annual evaluation is completed and signed, unless circumstances warrant otherwise.
Supervisor's File. Files maintained by supervisors regarding an employee are considered part of the employee’s personnel file and subject to the requirements of state law, RCW 49.12.240, RCW 49.12.250, RCW 49.12.260, and any provisions of this Agreement applicable to personnel files, including allowing employee access to such files.
Supervisor's File. Nothing in this Agreement is to be construed to restrict the supervisor of an employee from keeping anecdotal records and supportive data for the process of annual evaluation.

Each employee shall have 40 the right, upon request, and after making an appointment with the supervisor, to review the 41 contents of his/her working file. The review shall be made in the presence of the supervisor or 42 his/her designee. During the review the employee shall be allowed to copy any material therein 43 at their own expense and shall be permitted to make a written inventory of material and, on 44 request, have inventory signed and dated by the supervisor. By August 31st of each year all 45 materials shall be destroyed or maintained in the personnel records at the district office.

  • EMPLOYEE FILES 10.01 A copy of any completed formal evaluation which is to be placed in an employee’s file shall be first reviewed with the employee. The employee shall initial such evaluation as having been read and shall have the opportunity to add her or his views to such evaluation prior to it being placed in her or his file. It is understood that such evaluations do not constitute disciplinary action by the Employer against the employee. Having provided a written request to the Director of Care, or her designate, an employee shall be entitled to her personnel file for the purpose of reviewing any evaluations or formal disciplinary notations contained therein, in the presence of the Director of Care, at a mutually agreeable time.
