SUPPLEMENTARY HEALTH CARE. The Plan provides coverage for active full-time employees which includes ambulance services, rental of wheel chairs and hospital equipment, private nursing (with a ten thousand dollar [$10,000.00] maximum every thirty-six (36) months), artificial limbs and braces. Also, included are services of a chiropractor, osteopath, naturopath, podiatrist, physiotherapist, speech therapist and masseur (each with a maximum of ten dollars [$10.00] per visit, thirty [30] visits per year). In addition, services of a psychologist (three hundred dollars [$300.00] per year) and hearing aids (three hundred dollars [$300.00] lifetime) are also covered. Dental treatment for accidental injury to the teeth and outside Canada coverage for physicians and surgeons are covered.
SUPPLEMENTARY HEALTH CARE. Employees may initially choose Option A or Option B or no coverage at all. If no coverage is chosen initially, only Option A will be available on January 1 of the next year. Option B can be selected in the following year. Regular part-time employees are eligible to participate in the supplementary health care plan if they work at least thirty (30) hours per week, reside in Canada and are covered by the provincial health care.

SUPPLEMENTARY HEALTH CARE. (a) The Employer will pay one hundred (100%) percent of the premium cost of a Supplementary Health Care Plan for regular employees.
(b) The Master Policy will govern the details of benefits and eligibility.
SUPPLEMENTARY HEALTH CARE. 100% of the premium cost for eligible employees and their eligible dependents for the following benefits:
a) Ambulance service
b) Up to $400 per calendar year for chiropractors, physiotherapists, podiatrists, and speech therapists c) Up to $400 every three (3) years for hearing aids
SUPPLEMENTARY HEALTH CARE. The Association agrees to participate in the University of Xxxxxx Xxxxxx Island Supplementary Health Care Plan, in accordance with the provisions of the Plan policy.

SUPPLEMENTARY HEALTH CARE. The Company’s Health Plan is designed to supplement the benefits provided by your provincial hospital and medicare programs. It helps pay the cost of most hospital and medical expenses incurred by you and your eligible dependents.
SUPPLEMENTARY HEALTH CARE. There will be a ten dollar ($10.00) yearly deductible for single and twenty dollar ($20.00) yearly deductible for family with ninety percent (90%) reimbursement thereafter. Semi private hospital is subject to the present terms and conditions of the policy in effect.
