Vision Benefit Plan Clause Samples

The Vision Benefit Plan clause outlines the specific vision care services and benefits provided to eligible individuals under an insurance or employee benefits agreement. Typically, this clause details what types of vision care—such as eye exams, prescription glasses, or contact lenses—are covered, the frequency of coverage, and any applicable copayments or limitations. By clearly defining the scope and terms of vision-related benefits, this clause ensures that both the provider and the beneficiary understand what vision services are included, thereby reducing confusion and helping manage expectations regarding coverage.
