VISION REIMBURSEMENT. The City will reimburse employees submitting a satisfactory receipt of purchase of vision services or products. The amount of the annual reimbursement will be based upon the employee’s benefit enrollment eligibility and are as follows:
VISION REIMBURSEMENT. ‌ Any bargaining unit employee who is not covered by the Kaiser or Providence vision plans and purchases corrective lenses or frames shall be reimbursed up to $85.00 per calendar year for said purchase. Proof of purchase of such lenses or frames shall be submitted to the Chief Human Resources Officer.
VISION REIMBURSEMENT. ‌ Paratransit Dispatchers covered by Xxxxxx’x vision benefit will not be entitled to the allowances under this article. Paratransit Dispatchers covered by the Providence Health Plan will use the Plan’s vision benefit as primary coverage and during alternate years be reimbursed as outlined under this article. Employees who waive medical insurance will receive the vision benefit outlined herein. Paratransit Dispatchers will be reimbursed for the exam, one set of lenses or contacts, and one pair of frames up to the maximum benefit allowance per year. The Employer will pay one hundred percent (100%) of the cost of the annual eye exam. New corrective lenses, frames, and contact lenses will be reimbursed when the eye examination shows a need for a change in the lens prescription. No reimbursement will be made for lost, stolen, broken, or cosmetic changes in eye wear which occur after a person has been reimbursed once per calendar year. Maximum vision care benefit payments are: Lenses - Up to $150.00 Frames - Up to $100.00 Contacts - Up to $250.00 An itemized receipt will be necessary in order to receive reimbursement. Receipts, an Optometrist’s Certificate, and a request for reimbursement can be submitted to the Chief Human Resources Officer for approval. Reimbursement will be made to the employee on the following payroll cycle.
VISION REIMBURSEMENT. The Employer shall pay a replacement cost equal to the vision care replacement cost, per incident, for prescription lenses/frames which are lost or damaged while performing City duties, provided that the employee shall submit to the Employer a written request for replacement, along with an explanation of occurrence and proof of loss.
VISION REIMBURSEMENT. Employees are eligible for vision care reimbursement subject to the following criteria: an employee is eligible for such reimbursement after six months continuous employment, working at least 50% or more time. Employees are reimbursed for the cost of either lenses and frames or contact lenses specifically prescribed for the employee only, up to a maximum of $150 in a twenty- four month period beginning January 1, 1998.

  • Educational Reimbursement SECTION 1. The purpose of this Article is to xxxxxx a learning environment and provide educational opportunities that are mutually beneficial to the employees and the County and will encourage eligible employees to participate in education programs which will further their skills and knowledge for use in their current position or for use in a possible future position of greater responsibility. The Educational Reimbursement Program shall be a plan as provided for in Section 127 of the Internal Revenue Code of 1986, as amended (the “Code”) and shall be construed consistently with the requirements of Section 127. Amounts paid for tuition reimbursement meeting the requirements of Section 127 of the IRS Tax Code are not included in an Employee’s income or subject to income tax withholding up to a maximum of $5,250 annually. If subsequent tax law changes fail to continue the tax-free treatment, or in any way modify its treatment, appropriate adjustments in tax withholding will be made from the effective date of the change. This Article does not apply to training seminars, conferences, workshops, etc.

  • Sick Leave Reimbursement An employee who has received sick leave benefits for injuries caused by a third party shall be obliged, in the event such employee undertakes an action for recovery of damages against the third party, to seek recovery of the total cost of wages and benefits paid to the employee while on sick leave. The employee shall be obliged to reimburse the Employer to the extent the employee succeeded in recovering such wages and benefits. This provision includes claims made to ICBC.

  • Mileage Reimbursement A. Subject to the current Vehicle Rules and Regulations established by the Board, an employee who is authorized to use a private automobile in the performance of duties shall be reimbursed for each mile driven in the performance of his or her duties during each monthly period as follows:
