Certification of Fitness. Prior to reinstating the employee, the Employer is entitled to require documentation, at the Employer's expense if the cost is not borne by the Health Care Plan, from a physician or from the Workers' Compensation Board, certifying that the employee is medically able to resume the performance of the duties.
Certification of Fitness. A pregnant unit member shall present to her supervisor a written statement by her physician or the practitioner of a well-organized church or denomination of her physical capacity to perform the duties of her position at the time she notifies the Board of her pregnancy. When, notwithstanding such certification of fitness, the performance of a pregnant unit member has substantially declined from the performance demonstrated by said employee at the time immediately prior to the time when notification was given of the state of pregnancy, the unit member may then be required to submit a physician’s statement or a statement of a practitioner of a recognized church or denomination that she is physically fit to perform the duties assigned to her.
Certification of Fitness. As a condition of taking a pregnancy disability leave or transfer, the unit member must provide medical certification from her health care provider that she is disabled due to pregnancy, childbirth or related medical conditions. However, pregnancy disability leave will not be denied in emergency situations or where the need for leave was otherwise unforeseeable, even if the unit member did not provide advance notice of the need for the leave or transfer. The certification should include the date on which the unit member became disabled due to pregnancy; and the probable duration of the period(s) of disability or the period(s) for the advisability of the transfer; and a statement that, due to the disability, the unit member is unable to work at all or to perform any one or more of the essential functions of her position without undue risk to herself, the successful completion of her pregnancy, or to other persons, or a statement that, due to pregnancy, the transfer is medically advisable. Recertification may be required where additional time is requested.

When, notwithstanding such certification of fitness, the performance of a pregnant unit member has substantially declined from the performance demonstrated by said unit member at the time immediately prior to the time when notification was given of the state of the pregnancy, or when the unit member has been absent more than three (3) consecutive days, the unit member may then be required to submit a physician's statement or statement of a practitioner of a well-recognized church or denomination that she is physically fit to perform the duties assigned to her.
Certification of Fitness. After an absence due to illness or injury, the Employer may require documentation and/or Certificate of Fitness from a physician or the WorkSafeBC, certifying that the employee is medically able to resume the full duties of the position or a modified return to work. The Employer agrees to pay for the Certificate of Fitness to a maximum of thirty dollars ($30) per incident.
Certification of Fitness. All material pre-existing health conditions and physical limitations of the participant will be disclosed by the participant or the participant’s parent or guardian in writing before beginning any activity. I have listed below any medical conditions which may hinder my abilities in the selected activities.

When, notwithstanding such certification of fitness, the performance of a pregnant classified employee has substantially declined from the performance demonstrated by said employee at the time immediately prior to the time when notification was given of the state of pregnancy, when the employee has been absent more than three (3) consecutive days, the employee may then be required to submit a physician's statement or statement of a practitioner of a well-recognized church or denomination that she is physically fit to perform the duties assigned to her. The District need not assume that an employee's statement of her physician's or practitioner's statement established physical capacity conclusively, but may require a review and examination by a physician selected by the District or a practitioner of the employee's church or denomination selected by the District. The cost of such reexamination, minus medical insurance reimbursement, shall be borne by the District. Refusal to submit certification of fitness as required by this provision shall be considered by the Board as grounds for dismissal.
Certification of Fitness. (c) Employees in receipt of WorkSafeBC benefits will be credited with seniority during any absence due to injury. Seniority for the purposes of this clause means the employee’s regularly scheduled hours during the term of their absence.

When, notwithstanding 31 such certification of fitness, the performance of a pregnant unit member has 32 substantially declined from the performance demonstrated by said unit 33 member at the time immediately prior to the time when notification was 34 given of the state of the pregnancy, or when the unit member has been 35 absent more than three (3) consecutive days, the unit member may then be 36 required to submit a physician's statement or statement of a practitioner of 37 a well-recognized church or denomination that she is physically fit to 38 perform the duties assigned to her. The District need not assume that the 39 unit member's statement or her physician's or practitioner's statement 40 establishes physical capacity conclusively but may require a review and 41 examination by a physician selected by the District or a practitioner of the 42 unit member's church or denomination selected by the District. Refusal to 43 submit certification of fitness as required by this provision shall be 44 considered by the Board as grounds for some form of disciplinary action.
