Consent to Audit. Business Associate shall give reasonable access to PHI, its internal practices, records, books, documents, electronic data and/or all other business information received from, or created or received by Business Associate on behalf of DSHS, to the Secretary of DHHS and/or to DSHS for use in determining compliance with HIPAA privacy requirements.
Consent to Audit. The Contractor shall give reasonable access to PHI, records, books, documents, electronic data and/or all other business information received from, or created or received by Contractor on behalf of the County and DSHS, to the Secretary of the U.S. Department of Health and Human Services or designee and/or to County for use in determining the County’s compliance with HIPAA privacy requirements.
Consent to Audit. Business Associate shall make its internal practices, books, records, and any other material requested by the Secretary relating to the use, disclosure, and safeguarding of PHI received from Covered Entity, available to the Secretary for the purpose of determining compliance with the Privacy Rule. The aforementioned information shall be made available to the Secretary in the manner and place as designated by the Secretary or the Secretary's duly appointed delegate. Under this Agreement, Business Associate shall comply and cooperate with any request for documents or other information from the Secretary directed to Covered Entity that seeks documents or other information held by Business Associate.
Consent to Audit. When applicable, at least annually, you will make yourself, your premises, your documentation and due diligence processes and procedures available for audit by us for compliance with the ACH Rules as well as mitigation of risk and regulatory compliance. We will provide you with no less than ten (10) days prior notice of any audit.
Consent to Audit. Business Associate shall make its records, books, documents, electronic data and all other business information available to the Secretary of the U. S. Department of Health and Human Services, the Office of the Inspector General, the Office of Civil Rights or to Covered Entity for review to confirm compliance with this Agreement and with federal and state law. If the Business Associate fails to comply with this provision, the Covered Entity, in its sole discretion, may immediately terminate this Agreement. Termination of this Agreement does not relieve Business Associate of the obligation to provide access to its records and other information as requested pursuant to federal law, to the same extent Covered Entity is required to make such records and information available.

The Bank may periodically conduct an audit for compliance with applicable laws, regulations, operating rules, including NACHA Rules, mitigation of risk and the terms of this Agreement. Customer agrees to cooperate by making itself, its premises, documentation and due diligence processes and procedures available to the Bank and to provide any other information that may be necessary or appropriate so that the Bank can complete the audit.

Contractor must give reasonable access to PHI, its internal practices, records, books, documents, electronic data, and/or all other business information received from, or created, received by Contractor on behalf of SBHASO or HCA, to the Secretary of the United States Department of Health and Human Services (DHHS) and/or to HCA for use in determining compliance with HIPAA privacy requirements.
Consent to Audit. You consent to an audit to confirm your compliance with the terms of this Agreement, on one week’s notice, on your property at reasonable times. You will comply in all respects with the reasonable requests of the auditor, making available for inspection your equipment, files, and accounting records relating to karaoke entertainment activities involving use of Sound Choice branded products. You agree that the audit coverage extends to any Sound Choice®-branded karaoke track in your possession. We agree that we will not audit you more than once per calendar year.

Business Associate shall give reasonable access to PHI, its internal practices, records, books, documents, electronic data and/or all other business information received from, or created or received by Business Associate on behalf of GCBH, DSHS, to the Secretary of DHHS and/or to DSHS for use in determining compliance with HI PAA privacy requirements.
5.1 O Obligations of Business Associate Upon Expiration or Termination. Upon expiration or termination of this Agreement for any reason, with respect to PHI received from GCBH, or created, maintained, or received by Business Associate, or any Subcontractors , on behalf of GCBH, Business Associate shall:
5.10.1 Retain only that PHI which is necessary for Business Associate to continue its proper management and administration or to carry out its legal responsibilities;
5.10.2 Return to GCBH or destroy the remaining PHI that the Business Associate or any Subcontractors still maintains in any form;
5.10.3 Continue to use appropriate safeguards and comply with Subpart C of 45 CFR Part 164 (Security Standards for the Protection of Electronic Protected Health Information) with respect to Electronic Protected Health Information to prevent Use or disclosure of the PHI, other than as provided for in this Agreement, for as long as Business Associate or any Subcontractors retain the PHI;
5.10.4 Not Use or disclose the PHI retained by Business Associate or any Subcontractors other than for the purposes for which such PHI was retained and subject to the same conditions set out in the "Use and Disclosure of PHI" section of this Agreement which applied prior to termination; and
5.10.5 Return to GCBH or destroy the PHI retained by Business Associate, or any Subcontractors, when it is no longer needed by Business Associate for its proper management and administration or to carry out its legal responsibilities.
