Dental Insurance Program. During the period of this AGREEMENT, the following dental insurance program shall be available for all regular full-time employees and all regular part-time employees under contract to perform service for at least six (6) hours per day who are covered by this AGREEMENT who complete the required applications for such insurance and transmit such applications to the Treasurer of the BOARD. Appropriate information and application forms will be provided to all new employees by the Treasurer at time of pre- employment processing, and it is desirable that such applications be completed and filed at that time if the employee desires insurance coverage. A notification regarding the annual open enrollment period will be posted in each school building prior to and during such annual open enrollment period. (For the purpose of the coverage specified in this Section, a "regular full-time employee" means any employee who is under contract to the BOARD who works at least six (6) hours per day on a regular basis and who is under Contract on a twelve (12) month basis. A regular part-time employee for such purposes is an employee who is under contract to the BOARD on any basis other than as a full- time employee basis.) Dental Expense Insurance, for employees and dependents of employees - Superior Dental Plan, or its equivalent secured from another carrier or self-insured in whole or in part by the BOARD.

Dental Insurance Program. 1. The current Dental Expense Insurance program for Teaching Staff Members and dependents outlined in a Memorandum of Understanding, is incorporated into this Agreement and shall be in full force and effect until a successor program is negotiated by the Discussion and Resolution Committee. 2. The dental insurance provided pursuant to this Agreement shall be subject to the conditions set forth in any insurance contract secured by the BOARD, provided, however, if the BOARD elects to change carriers, any new insurance coverage secured shall be equivalent to the coverage described in a Memorandum of Understanding, including the conditions set forth in the insurance contract. to any month such coverage is desired to be continued. Full time Teaching Staff Members on approved disability leave of absence of over ninety (90) days duration who desire to continue insurance coverage for the Teaching Staff Member may do so by paying one-half (1/2) of the full group insurance premium for such insurance to the Treasurer of the BOARD on or before the seventeenth (17th) day of the month prior to any month such coverage is desired to be continued for the remainder of the first year of the disability leave. In the case of Teaching Staff Members who work less than full-time the Board shall pay one-half (1/2) of the premium it pays when such Teaching Staff Member is on the active working payroll. In the event such part premium is timely paid by the Teaching Staff Member, the BOARD will be responsible for the remaining one-half (1/2) of the full group premium for the remainder of the first year of disability leave. In the event coverage is discontinued for any period, the Teaching Staff Members shall have the right to acquire insurance through the insurance carrier in accordance with its policies and coverage cannot be reacquired through the BOARD until the Teaching Staff Member returns to active working status. D. For the purpose of this ARTICLE, the effective date of the resignation of any Teaching Staff Member shall be either: (1) the effective date of resignation as stated in the letter of resignation, if such effective date occurs during the regular instructional year, or, (2) the day prior to the commencement of the next school year where the instructional year has been completed by the resigning Teaching Staff Member. Dental Insurance Program. The Board shall provide a Dental Insurance Program (Advantage Plus Plan) with Delta Dental. The Board shall pay full premium for this dental insurance program for full family coverage. This coverage will provide a maximum yearly benefit of $1,500 for each family member covered by the plan.

  • Dental Insurance The State agrees to pay one hundred percent (100%) of the employee premium of a dental insurance program for full-time employees. The benefit levels of this program shall provide one hundred percent (100%) coverage for preventive care and eighty percent (80%) coverage for general service care. The State agrees to provide payroll deduction for dental insurance, provided such arrangements are agreed to by the insurance carrier. Dependent coverage will be available provided there is sufficient employee participation in the dental insurance program. Dependent coverage will be at the employees' expense.

  • Group Dental Insurance Not available to part-time Station Attendants. Group insurance coverage for temporary full-time employees will be in accordance with XXX #1. Such benefits, once established, are retained even if an employee's status reverts back to part-time, providing that employment has been continuous.

  • Health and Dental Insurance ☐ Husband ☐ Wife shall maintain coverage for each minor child under the medical and dental insurance provided through his/her employment. To facilitate the use of such coverage for the child(ren), the Couple shall cooperate fully and in a timely manner, including, but not limited to, obtaining and providing all necessary insurance cards and claim forms, completing and submitting all necessary documents, and delivering all insurance payments.   • Insurance Program An eligible employee may waive rights to participate in either single or family coverage. If an employee waives this benefit, such employee may not revoke the waiver until the next open enrollment period and may be accepted only after medical review by the insurance provider.

  • Group Insurance Plan The carriers, coverage, and terms and conditions of participation under the District’s Group Insurance Plan are subject to change in accordance with the applicable provisions of Title I, Division 4, Chapter 10 of the California Government Code (Section 3500 et seq.) (Xxxxxx‐Milias‐Xxxxx Act). a. The District contracts with CalPERS for health plan coverage for all regular and newly hired employees (eligibility to be defined by the “CalPERS health plan”). Booklets on the insurance plans will be available to all participants. b. Employees may choose from the available plans offered by CalPERS. Additional premiums will be borne by the employee through payroll deductions and paid to CalPERS by the District each month; and the additional cost for monthly premiums will be deducted evenly from the first and second payroll period of each month.
