Dental/Vision Plan. If and when the trustees of the existing dental plan request the City for additional money to fund the Health and Welfare Fund, the employer will contribute up to $13 per week per employee to the City Health & Welfare Trust Fund. Should the trustees of said fund inform the employer and the Union that the fund requires additional payments; the cost of these payments above $13.00 (thirteen dollars) per week per member shall be shared on a fifty percent (50%) basis between the employer and the employee.

Dental/Vision Plan. The Employer shall provide reimbursement for dental and vision expenditures for each full-time employee and his dependents in the following amounts: For those married employees with no children and both working in the District they shall each receive the individual rate which will be put in a joint “pot” upon which both can draw. For those married employees with children and both working in the District, one employee shall receive the individual rate and the other employee will receive either 2- party, if one child, or family rate if more than one child. Said monies will be put into a “family pot” upon with the entire family can draw upon.
Dental/Vision Plan. A dental plan associated with the City Plan or an alternative approved by the Superintendent and School Committee will be offered effective July 1, 1983. Effective January 15, 2001, if requested by the trustees of the existing dental plan, the employer shall contribute up to $13.00 per member per week, and should the trustees of said plan inform the employer and the union that the fund requires additional payments, the cost of these payments above $13.00 (thirteen dollars) per week per member shall be shared on a fifty percent (50%) basis between the employer and the employee.

  • Group Insurance Plan The carriers, coverage, and terms and conditions of participation under the District’s Group Insurance Plan are subject to change in accordance with the applicable provisions of Title I, Division 4, Chapter 10 of the California Government Code (Section 3500 et seq.) (Xxxxxx‐Milias‐Xxxxx Act). a. The District contracts with CalPERS for health plan coverage for all regular and newly hired employees (eligibility to be defined by the “CalPERS health plan”). Booklets on the insurance plans will be available to all participants. b. Employees may choose from the available plans offered by CalPERS. Additional premiums will be borne by the employee through payroll deductions and paid to CalPERS by the District each month; and the additional cost for monthly premiums will be deducted evenly from the first and second payroll period of each month. To the extent allowed by law, the District will attempt to deduct the employee’s premium contribution from pre‐tax dollars.

  • 401(k) Plan Executive shall be entitled to participate in the Company’s 401K plan in accordance with its terms and conditions.

  • Insurance Plan 19.01 The Employer agrees to contribute the indicated percentage of the premium cost of the following group plans for full-time employees (and their families where applicable) who have completed their probationary period.

  • Medical Plan ‌ Eligible employees and dependants shall be covered by the British Columbia Medical Services Plan or carrier approved by the British Columbia Medical Services Commission. The Employer shall pay one hundred percent (100%) of the premium. An eligible employee who wishes to have coverage for other than dependants may do so provided the Medical Plan is agreeable and the extra premium is paid by the employee through payroll deduction. Membership shall be a condition of employment for eligible employees who shall be enrolled for coverage following the completion of three (3) months’ employment or upon the initial date of employment for those employees with portable service as outlined in Article 14.12.
