Dialysis. The Plan provides Benefits for Medically Necessary hemodialysis and dialysis on an Inpatient or Outpatient basis, or at home. When the Member is eligible for coverage of hemodialysis and dialysis under Medicare, the Plan provides Benefits only to the extent payments would exceed what would be payable by Medicare. Your PCP or kidney specialist should make all arrangements for hemodialysis and dialysis care. Coverage for hemodialysis and dialysis in the home includes nondurable medical supplies, drugs, and equipment. To be covered, hemodialysis and dialysis services under this section must be ordered by a Physician.
Dialysis. Benefits will be provided for Medically Necessary dialysis services and supplies when Referred by the PCP. In the case of home dialysis, services include equipment, training, and medical supplies. The decision to purchase or rent necessary equipment for home dialysis will be made by Keystone.

Dialysis. Ownership, operation and management of -------- facilities and businesses which provide hemodialysis services and treatments to patients with chronic or acute kidney diseases or conditions;
Dialysis. The treatment of acute renal failure or chronic irreversible renal insufficiency for removal of waste materials from the body by hemodialysis, peritoneal dialysis, hemoperfusion, or chronic ambulatory peritoneal dialysis (CAPD), or continuous cyclical peritoneal dialysis (CCPD).
Dialysis. Expenses concerning dialysis – including all doctor, room-meal-companion, medication, diagnosis unit expenses – regardless of whether they are inpatient or outpatient, will be paid within the limit specified in the policy and the special and general conditions.
Dialysis. If the following criteria are met, we cover dialysis Services related to acute renal failure and chronic end-stage renal disease (ESRD): 1. You satisfy all medical criteria developed by Medical Group and by the facility providing the dialysis; and 2. A Plan Physician provides a written referral for care at the facility. We cover the following renal dialysis Services: 1. Outpatient maintenance dialysis treatments in a Plan dialysis facility. Coverage includes the cost of laboratory tests, equipment, supplies and other Services associated with your treatment; 2. Inpatient maintenance dialysis if you are admitted to a Plan Hospital because your medical condition requires specialized hospital Services on an inpatient basis; and 3. Plan Provider Services related to inpatient and outpatient dialysis. We cover the following self-dialysis Services: 1. Training for self-dialysis including the instructions for a person who will assist you with self-dialysis; 2. Services of the Plan Provider who is conducting your self-dialysis training; and 3. Retraining for use of new equipment for self-dialysis. We cover home dialysis, which includes: 1. Hemodialysis; 2. Home intermittent peritoneal dialysis (IPD); 3. Home continuous cycling peritoneal dialysis (CCPD); and 4. Home continuous ambulatory peritoneal dialysis (CAPD). Members traveling outside the Service Area may receive pre-planned dialysis Services for up to sixty (60) days of travel per calendar year. Prior authorization is required. Independently provide and monitor haemodialysis treatment including providing pre-and post- haemodialysis care. • Appropriately respond to clinical changes or deterioration in the consumer's condition and initiate consultations with relevant caregivers as required. • Provide comprehensive educating to consumers following their treatments including home care needs, diet, nutrition, exercise programs and any self-administration medication. Aged Care • Demonstrate knowledge and best practice principals for continence management, diabetes management, palliative care, wound management. • Manage clinical risks such as hydration, nutrition, pain, pressure, wound, and delirium. • Facilitate and encourage social connections between consumers and their health care team, and families. • Collaborates with the services multidisciplinary team to achieve the best health outcomes for our consumers. • Care plans are comprehensive and at minimum meet legislative and standards requirements. • Consumers are encouraged and supported to participate in lifestyle activities. After Hours Hospital Coordination • The most senior clinician on site and represents YDHS and the YDHS Senior Management Team members professionally to consumers, caregivers, and our community. • Facilitate safe and seamless care to consumers across the health service. • Manage demand, capacity, and patient flow. • Facilitate and support appropriate consumer transfers. • Act as the emergency response coordinator. • Manage organisational risks by collaborating with relevant teams to ensure interdisciplinary problem solving. • Management of the unit staff roster. • Utilising available data to develop a roster of adequately skilled staff. • Ensure caregiver/patient ratios meet the needs of the unit. • Ensure Caregivers understand their roles and responsibilities within their scope of practice. • Promote appropriate behaviour and create an environment of accountability. • Facilitate education as required to ensure consumer safety.
