Disability and Life Insurance. Subject to the provisions contained in this Article, Exhibits D and E, the applicable insurance and agency contacts, the City shall pay premiums for providing the following insurance coverage for employees commencing with the City's billing date for such insurance immediately following the completion of thirty (30) days of continuous service:

Disability and Life Insurance. Executive will be eligible for short-term and long-term disability insurance and Company-paid Group Term Life Insurance equal to three times his annual earnings up to a maximum of $1,000,000.00 and AD&D Insurance upon hire. These benefits are provided at no cost to Executive.
Disability and Life Insurance. (a) Company will provide at Company’s expense, and for the benefit of Executive and his designated beneficiaries, disability and life insurance for the duration of the Employment Term in the amount of at least €1,000,000.00 in the event of disability and at least €500,000.00 in the event of death, all as provided to Executive immediately prior to the effective date of this Agreement.

Long-Term Disability The Company is responsible for the administration of the Long Term Disability (LTD) Plan and agrees that there will be no unilateral reduction in benefit levels. Employee benefits brochures shall be provided by the employer to all employees who are eligible for benefits, at time of hire or upon request. The employees will pay 100% of the LTD premiums.

Disability and Life Insurance. (ACGME Institutional Requirement IV.C.2.h., and IV.G.2. and 2.a.) Disability and life insurance are provided for Residents/Fellows through the GME designated carriers and may not be cancelled during residency/Fellowship training. The Department of GME provides a stipend to offset the cost of the life and disability insurance. Per ACGME Requirements, as with health insurance, Residents/Fellows are covered by the long term disability insurance as of the first day of employment.
