Disability and Life Insurance. Subject to the provisions contained in this Article, Exhibits D and E, the applicable insurance and agency contacts, the City shall pay premiums for providing the following insurance coverage for employees commencing with the City's billing date for such insurance immediately following the completion of thirty (30) days of continuous service:
Disability and Life Insurance. Short-term and long-term disability Insurance, Company-paid Group Term Life Insurance equal to three times Executive’s annual earnings up to a maximum of $1,000,000.00, and AD&D Insurance will be provided at no cost to Executive.
Disability and Life Insurance. (a) Company will provide at Company’s expense, and for the benefit of Executive and his designated beneficiaries, disability and life insurance for the duration of the Employment Term in the amount of at least €1,000,000.00 in the event of disability and at least €500,000.00 in the event of death, all as provided to Executive immediately prior to the effective date of this Agreement.
(b) Company will have the right to take out life, health, accident, “key-man” or other insurance covering Executive, in the name of Company and at Company’s expense in any amount deemed appropriate by Company. Executive will assist Company in obtaining such insurance, including, without limitation, submitting to any required examinations and providing information and data required by insurance companies.
Disability and Life Insurance. HOSPITAL will provide disability insurance and life insurance at no cost to PROFESSIONAL during the term of this Contract. Additionally, PROFESSIONAL shall be covered by HOSPITAL’s workers’ compensation program to the extent that PROFESSIONAL suffers a work related injury or disability.
Disability and Life Insurance. NCH shall provide Resident with such basic life, short and long term disability insurance as NCH provides for its other employees. Coverage for benefits will begin as stated in the Employee Manual.
Disability and Life Insurance. Full-time employees shall be entitled to participate in the Long Term Disability and the Life and Accidental Death/Dismemberment insurance program currently in effect. The Short Term Disability Insurance plan provided by the City is mandatory for all employees in this unit, with the cost to be borne by the City. The death benefit for any Life Insurance program shall be fifty thousand dollars ($50,000.00), at no cost to the employee.
Disability and Life Insurance. Short-term and long-term disability insurance, Company-paid Group Term Life Insurance and AD&D Insurance will be provided at no cost to Executive.
Disability and Life Insurance. (a) Company shall pay for or reimburse Employee the cost of a disability insurance policy which shall provide the highest rate of compensation then available, but in no event less than Two Hundred Thousand and No/100 Dollars ($200,000.00) per year, a ninety (90) day waiting period, and benefits payable through the age of seventy-five (75).
(b) Company shall pay for or reimburse Employee for a life insurance policy which will provide a death benefit in the amount of Two Million and No/100 Dollars ($2,000,000.00), which shall contain a cost of living adjustment endorsement for each calendar year during the term of this Agreement.
(c) Provided, however, that the total annual cost of such disability and life insurance coverage for Employee to be paid by the Company shall not exceed Two Thousand Dollars ($2,000) per year. Any cost of such insurance in excess of $2,000 per year shall be paid by Employee.
Disability and Life Insurance. (ACGME Institutional Requirement IV.C.2.h., and IV.G.2. and 2.a.) Disability and life insurance are provided for Residents/Fellows through the GME designated carriers and may not be cancelled during residency/Fellowship training. The Department of GME provides a stipend to offset the cost of the life and disability insurance. Per ACGME Requirements, as with health insurance, Residents/Fellows are covered by the long term disability insurance as of the first day of employment.
