HEALTH, DISABILITY AND LIFE INSURANCE. Subject to satisfaction of the eligibility requirements of such plans and the rules and regulations applicable thereto, Executive and his family members shall be entitled to be covered by the Company's group health and dental insurance plans presently in effect or hereafter adopted by the Company and applicable to employees of the Company generally and Executive shall be entitled to be covered by the Company's group disability and life insurance plans presently in effect or hereafter adopted by the Company and applicable to the employees of the Company in general. The Company shall pay the premiums associated with such coverage. In the event Executive makes a claim against any disability policy provided to Executive by the Company pursuant to this Section 5(c)(i) and such policy calls for a waiting period which is applicable to Executive's claim, the Company shall pay to Executive during such waiting period his monthly base salary due during such period and shall provide the other benefits due him under this Section 5(c)(i).
HEALTH, DISABILITY AND LIFE INSURANCE. The University will provide health, disability, and life insurance benefits to Residents/Fellows and their families as described in the GME Leave and Summary of Resident/Fellow Benefits Policy. Residents/Fellows have the option to decline this benefit.

NGMC will provide health, dental and vision care insurance to Resident and his or her eligible spouse and children in accordance with NGMC policies in effect from time to time. Residents are provided with life and disability insurance. Insurance benefits are subject to and will be provided in accordance with NGMC’s employee benefits handbook in effect from time to time. HEALTH, DISABILITY AND LIFE INSURANCE. All provisions of the Town personnel rules and regulations relating to health, vision, dental, and life and disability insurance as existing on July 1, 2024, or thereafter as may be amended, are incorporated herein by reference. EMPLOYER shall pay the amount of premium due for supplemental term life insurance in the amount of Three Hundred Fifty Thousand dollars ($350,000). EMPLOYEE shall name the beneficiary of the life insurance policy.
HEALTH, DISABILITY AND LIFE INSURANCE. A. Employer agrees to provide for health, hospitalization, surgical, vision, dental and comprehensive medical insurance for the Employee and his/her dependents equal to that which is provided to all other non-union Employees. Employer share of cost of health and dental insurance shall be the same as other non-union Employees. Employer agrees to allow the Employee to waive health insurance coverage through the City when provided with proof of coverage under another comprehensive plan. If Employee opts out of health insurance, Employer shall pay Employee the same opt out payment provided to all other non-union Employees.
B. The Employer agrees to put into force and to make required premium payments for short- term and long-term disability coverage for the Employee.
C. The Employer agrees to purchase, keep in effect, and pay the required premiums on a term life insurance policy equal to twice Employee’s annual salary. Said policy is to be owned by the Employee.
HEALTH, DISABILITY AND LIFE INSURANCE. 1. The Employer agrees to provide and to pay the premiums for health, hospitalization, surgical, dental, and comprehensive medical insurance for the Employee and his/her dependents equal to that which is provided to all other employees of the City of West Burlington.
2. The Employer shall pay the amount of premium due for term life insurance in the amount of $100,000.00. The Employee shall name the beneficiary of the life insurance policy. In addition, the City will provide Accidental Death and Dismemberment coverage in the amount of $100,000.00.
HEALTH, DISABILITY AND LIFE INSURANCE. Employee shall be entitled to receive all insurance benefits provided to all eligible employees of the City. The City shall pay the same premium for Employees’ policies that the City pays for all eligible employees of the City.
HEALTH, DISABILITY AND LIFE INSURANCE. The University will provide Health, disability, and life insurance benefits to Residents and their families as described in the Graduate Medical Education Policy and Operations Manual. Residents should refer to the Graduate Medical Education Policy and Operations Manual to familiarize themselves with the conditions of these benefit plans.
HEALTH, DISABILITY AND LIFE INSURANCE. Employee shall be entitled to participate during the Term in any group health or salary continuation or life insurance plans made available by Employer for its employees in the jurisdiction in which Employee is performing services for Employer, subject to the terms, conditions and eligibility restrictions set forth in such plans as from time to time in effect. The premiums for coverage shall be paid by Employer and Employee in such proportion as may be determined from time to time by Employer.
