Eligibility for Insurance Coverage. 1. Subject to the provisions of P.A. 152 of 201 I, the Employer has chosen the 80%/20% arrangement. The Employer shall provide 80% of the medical premium. The MESSA ABC Plan I, $1,300-single deductible, $2,600-double, and family deductible from January 1, 2017. After year one of the collective bargaining agreement the district may provide a substantially equivalent benefit package beginning July I, 2018, by mutual agreement. Lincoln Consolidated Schools shall be the policy holder. Employees are responsible for 20% of the premium costs of individual health insurance. An employee must be assigned and work a regular schedule of twenty (20) to forty (40) hours or more per week to receive this benefit. By mutual agreement the parties may substitute an equivalent plan. Time driven beyond regular assignments will not be considered in computing time for purposes of this Section. Employees assigned and working less than 20 hours per week (less than 4 hours per day) shall not normally be eligible for healthcare insurance coverage unless they pay 100% of the costs incurred in joining the MESSA A Group Plan. Employees hired after March 1, 2016 will require a scheduled work week of thirty (30) hours or more to qualify for any Employer provided healthcare insurance. For purpose of this Agreement any and all hours regularly scheduled within the District will count for thirty (30) hour minimums for fringe benefit qualifications.

Eligibility for Insurance Coverage. 1. The employer will pay up to the full premiums for insurance coverage for transportation employees (excluding aides) who were assigned and working a regular transportation schedule of twenty (20) hours or more a week as of December 30, 1987.
Eligibility for Insurance Coverage. 1. Subject to the provisions of P.A. 152 of 2011, the Employer has chosen the hard cap arrangement. The Employer shall provide and pay the costs, up to the hard cap amounts, and of including payment of the deductible amounts, the Messa ABC Plan 1, $1,300- single deductible, $2,600 - double, and family deductible from January 1, 2015. This plan is purchased through the Consortium Agreement between the Employer and other member school districts. Lincoln Consolidated Schools shall be the policy holder. Prior to January 1, 2015, and as soon as possible after ratification, the Employer shall provide and pay the costs up to the P.A. 152 hard cap amounts of the Messa Plan PAK C 666C - WCC ($300/$600 deductible). Also available, only until January 1, 2015, the Messa Plan Choice 2 PAK A 666C-WCC ($200/$400 deductible), with the Employer paying the costs only to P.A. 152 hard cap amounts. An employee must be assigned and working a regular schedule of 20 to forty 40 hours or more per week to receive this benefit. By mutual agreement the parties may substitute an equivalent plan. Employees are responsible for any and all costs of individual health insurance which exceeds the hard cap.

  • Insurance Coverage The Company and each Subsidiary maintains in full force and effect insurance coverage that is customary for comparably situated companies for the business being conducted and properties owned or leased by the Company and each Subsidiary, and the Company reasonably believes such insurance coverage to be adequate against all liabilities, claims and risks against which it is customary for comparably situated companies to insure.

  • Required Insurance Coverage As a condition of this Contract with DIR, Vendor shall provide the listed insurance coverage within 5 business days of execution of the Contract if the Vendor is awarded services which require that Vendor’s employees perform work at any Customer premises and/or use employer vehicles to conduct work on behalf of Customers. In addition, when engaged by a Customer to provide services on Customer premises, the Vendor shall, at its own expense, secure and maintain the insurance coverage specified herein, and shall provide proof of such insurance coverage to the related Customer within five (5) business days following the execution of the Purchase Order. Vendor may not begin performance under the Contract and/or a Purchase Order until such proof of insurance coverage is provided to, and approved by, DIR and the Customer. All required insurance must be issued by companies that have an A rating and a Financial Size Category Class of VII from A.M. Best, and are licensed in the State of Texas and authorized to provide the corresponding coverage. The Customer and DIR will be named as Additional Insureds on all required coverage. Required coverage must remain in effect through the term of the Contract and each Purchase Order issued to Vendor there under. The minimum acceptable insurance provisions are as follows:

  • Subcontractor Insurance Coverage Contractor shall require and verify that all subcontractors maintain insurance coverage that meets the minimum scope and limits of insurance coverage specified in this Exhibit C. EXHIBIT D
