Follow-Up Visits. CORE teams shall make all reasonable efforts to complete Follow-up Visits to each CORE eligible household monthly for 3 months beginning March 23, 2017; every other month thereafter through December 31, 2017; and then once every 6 months through December 31, 2018.

If an employee or the mental health provider feel that services beyond the one (1) required appointment would be needed or beneficial for the employee, the mental health provider may create a referral at the employee’s discretion. These additional appointments are not mandatory. If the employee wishes to schedule follow up appointments with the mental health provider, the employee may schedule those appointments directly with the mental health provider.
a. The Police Department will pay for up to six (6) total appointments with the approved mental health provider for each employee per year. After the six (6) appointments, the employee is responsible for the payment of any further appointments; employees are encouraged to utilize their Health Insurance or Healthcare Reimbursement Account for any desired additional services or appointments.
Follow-Up Visits. Subsequent office visits will be billed separately if you return to DHC and/or NDHC.
Follow-Up Visits. You will be asked to return the following Thursday for further evaluation. When you become an established patient you will be seen on the or Thursday of every month. Thursday at 9:00am for the Xxxxx Hwy Clinic and 2:00pm the Pace location. I f you see you are going to miss your appointment you must call as soon as possible, no less than 24 hours. When you call the office make a note of the date, time, person you spoke to and the code they will give you. You wil l need this information when you come in and i f you do not have the right code, the call never happened. for some reason you are going to be going out of town at the time of your scheduled appointment you need to make an to see Dr. on the appropriate date and time before you go, so you not be with out your medicine for that time period, i.e. I am going to be leaving town for work the Sunday before I am due to come in on Thursday for my regular appointment and will be gone out of town for 3 weeks. So I call Dr office and schedule an appointment for the Thursday before I am to leave even though I still have medicine When I go to Xx Xxxx'x office I will inform him am going to be gone out of town for 3 weeks and will only have enough medicine for about a week. He determine when you will need to come back after you return to town. He might only give you enough medicine to cover the- time you will be gone which in this case is days worth of your medicine so you would have to pay for days at $6.00/day = or he might give you the hole months worth and charge you the month.
Follow-Up Visits. Follow-up visits, if required, will be billed as additional services, approved in advance by the Client.
Follow-Up Visits. ADOPTER understands and agrees that RESCUE is fully entitled to make follow up visits or phone calls to ascertain that all of the covenants of this agreement are being satisfied. ADOPTER is obligated to cooperate with RESCUE in setting and keeping appointments.
Follow-Up Visits. All welcome placements must be visited within the first thirty (30) days following the student’s arrival in the home, and every subsequent thirty (30) days after the initial visit was made, providing that the given student(s) is/are still in the same home.
i. It is the Local Coordinator's responsibility to check his/her AYA Local Coordinator Portal regularly to confirm Second Visits/ Follow Up requests and complete the reports for confirmed visits on or before the deadline indicated, as required by the Dept. of State.
ii. The Local Coordinator agrees to facilitate such visits for students outside his/her area where possible, and to act in accordance with all forthcoming information from the AYA National Office regarding these visits.

If you cancel a follow-up visit: • With less than 48 hours’ notice; or • On the day of your scheduled appointment; or • Fail to show for your appointment without notification, you will be charged £30. This fee will need to be paid before you rebook another appointment at our clinic.

Regretfully, we now enforce this policy due to last-minute cancellations, scheduling changes, and “no-shows.” We have a very busy practice. Assuring that all new and established patients have access to care when necessary is our priority. Cancelling or rescheduling with sufficient advanced notice makes it more likely that another patient in need will be able to use your time-slot. When you cancel or reschedule at the last minute, or fail to show for your appointment, you are depriving another patient the care they need. Patient appointments require us to block out large time slots, thus making last-minute cancellations and rescheduling of visits challenging and sometimes problematic. We spend an inordinate amount of time and energy with each and every one of our patients because we are committed to providing the highest quality care to be found anywhere.
