Health and Hospitalization Insurance – Single Coverage. The School District shall contribute a sum not to exceed $284.00 per month toward the premium for individual coverage for each full-time employee employed by the School District who qualifies for and is enrolled in single coverage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.
Health and Hospitalization Insurance – Single Coverage a. The School District shall contribute 100% of the premium for the 2019-2022 school years not to exceed $775 monthly for all full-time employees employed by the School District who qualify for and are enrolled in the School District group insurance and hospitalization plan. Any additional cost of the premium shall be borne by the employee and shall be paid by payroll deduction.
Health and Hospitalization Insurance – Single Coverage. The school district shall contribute a sum of not to exceed $1,850 per contract year ($154.17 per month) toward the premium for individual coverage for each eligible employee employed by the school district who qualifies for and is enrolled in the school district group health and hospitalization plan as provided herein. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction. Employees employed at Little Giant’s who work a minimum of 1000 hours are eligible for School District contribution toward single insurance on a pro rata basis. An employee presently employed as of 6/30/94 and enrolled in the group insurance plan shall only be eligible for School District contribution towards single coverage health and hospital insurance, up to the amount of $6,912 per year.
Health and Hospitalization Insurance – Single Coverage. For Paraprofessional and Food Service employees the School District shall contribute a sum of not to exceed $2,873.00 annually toward the premium for individual coverage for each 9-month employees employed by the School District who qualifies for and is enrolled in the School District’s group health and hospitalization insurance plan. Administrative Assistants of the bargaining unit shall be granted $3300.00 towards the premium for individual coverage. Health insurance premiums paid by the school district shall be done so over a nine (9) month period. Any additional cost of the premium shall be borne by the employee.
Health and Hospitalization Insurance – Single Coverage. The School District shall contribute the amount indicated below toward the premium for individual coverage for each Category 1 and 2 employee who qualifies for and is enrolled in single coverage in the School District’s group health and hospitalization insurance plan. Any additional cost of the premium shall be borne by the employee and paid by payroll deduction.
Health and Hospitalization Insurance – Single Coverage. The School District shall contribute a monthly sum not to exceed 90% of the $3000 deductible premium cost toward the premium for individual coverage for each full-time Employee employed by the School District who qualifies for and is enrolled in single coverage in the School District’s group health and hospitalization insurance plan. The School District contribution will be reduced by an employee/policyholder initial contribution of $75 each month. The School District contribution can be made toward either the $3000 or $5000 single plan. If the School District contribution(s) exceed premium costs, no reimbursement will be made to the Employee. Any additional cost of the premium shall be borne by the Employee and paid by payroll deduction.
Health and Hospitalization Insurance – Single Coverage. The School District shall contribute a sum not to exceed $808 per month for the first 6 months of the contract (July-Dec 23), then contribute a sum not to exceed $823 per month for all of calendar year 2024, then contribute a sum not to exceed $922 per month the last 6 months of the contract (Jan-June 25) toward the the premium for individual coverage for each full time teacher employed by the School District, who qualifies for and is enrolled in the School District’s group health and hospitalization plan. Any additional cost of the premium shall be borne by the teacher and paid by payroll deduction.
