Hospitalization Plan. A six-member committee will be appointed to select the carrier for the hospitalization plan. Three members will be appointed by the WEA and three members by the Board. However, notwithstanding any recommendation from this committee, the Board of Education, in its sole discretion, will have absolute authority to select not only the carrier, but the hospitalization plan to be made with a cap of a $5000 deductible on a standard insurance plan. The Winner School District will pay the single premium for the school’s $1500/$2000/$5000 deductible hospitalization plan, carried by the Associated School Boards for each Member of the Bargaining Unit up to $535.00 In the event that the single premium increases above the District’s maximum obligation, the WEA may seek from the carrier an increase in the deductible, but such changes shall not increase the District’s obligation under this provision. Members of the Bargaining Unit may elect to use Plans A or Plan B or Plan C in accordance with the Winner School District’s Schedule of Benefits. Members who are of Medicare Eligibility Age may elect to receive ½ of the total single premium towards a Medicare Supplement Plan in lieu of the hospitalization plan. This is a taxable event.

Hospitalization Plan. Section 50.1 The areas of coverage will be as follows: Section 50.2 The bargaining unit member will agree to pay 15% of the monthly premium cost. The City will agree to pay the balance of the monthly premium. Section 50.3 The City of Cambridge agrees to maintain the current dental program at no additional cost to bargaining unit members. Section 50.4 The City shall establish an Insurance Committee comprised of one (1) representative from each of the City’s employee bargaining units. These representatives must be enrolled in the City’s health care benefits. The City may appoint up to three (3) representatives who are also enrolled in the City’s health care benefits. For purposes of this article, Rank and patrol officers shall be considered one (1) unit, Dispatchers shall be considered one (1) unit, AFSCME shall be considered one (1) unit, and IAFF shall be considered one (1) unit. Each local Union shall notify the City of the names of its representative. Decisions of the committee shall be by a majority vote of the committee. Section 50.5 The Health Insurance Committee shall meet no later than sixty (60) calendar days prior to the end of the plan year to make decisions for the following plan year. The City will provide the Committee with all costs and experience data it has available. Section 50.6 The Health Insurance Committee shall have full control of any and all changes and/or adjustments to the City’s health care plan.

Should the Township elect to change PA 152 compliance options from the current status as of the date of this agreement, either party may request to reopen Section 13.l for further negotiation during the term of the agreement. In the event the parties do not reach an agreement, the matters subject to bargaining in 13.1 shall be resolved through arbitration pursuant to Act 312 of 1969, as amended. 13.2: The Township shall provide and assume the full cost of a hospital, medical, and surgical insurance policy for each employee who retires from the Department after twenty-five years of service (including enhanced service time). The policy shall cover the retired officer, the retiree's spouse and any dependent children. The coverage shall be the same CB 10 PPO - Option D as that provided to officers in 13.1 above. An employee, who retires meeting the above requirements but having service time (including enhanced service time) of less than twenty-five (25) years, may elect to receive this coverage with premiums to be funded as follows: Years of Service Paid by Employer Paid by Retiree 10-14 years 25% 75% 15-19 years 50% 50% 20-24 years 75% 25% 25-more years 100% 0% If an employee receives a duty disability pension, he/she shall be entitled to full hospitalization insurance at no charge regardless of years of service. Payment must he made to the Township twenty (20) days in advance of the month of applicable coverage. This coverage must be elected by the employee prior to the time of retirement and its continuation is voluntary by the retiree. Coverage must be maintained on a continuous basis except as provided in #5 below. A retiree who fails to make necessary premium payments timely may be disqualified for future coverage hereunder by the Township Board of Trustees.
