HOSPITALIZATION AND MEDICAL CARE INSURANCE FOR MAINTENANCE AND CUSTODIAL EMPLOYEES. (a) Effective July 1, 2012 the Employer shall provide regular full-time employees medical insurance not to exceed the following amounts: The following table illustrates the maximum board subsidy for health insurance. The board subsidy will not exceed the amounts listed below. Effective July 1, 2012 Total Board Maximum * Annual Single $5,500 Two-Person $11,000 Full Family $15,000 Cash in Lieu $4,322 * The Board Maximum will adjust annually at renewal as defined by 2011PA 152. All regular full-time employees newly hired on or after the date of ratification of this agreement are eligible for Single only health benefits. All current part- time employees on the date of ratification of this agreement will be eligible for up to Full Family health benefits on the month following the employee transferring into a full-time position. If a husband and a wife are both members of the bargaining unit, one shall select health insurance coverage and the other optional coverage and/or cash-in-lieu as defined above.

No premium subsidy shall be available to employees regularly scheduled to work less than thirty- two (32) hours per week.

  • Medical Benefits - Prescription Drugs Administered by a Provider (other than a pharmacist) This plan covers prescription drugs as a medical benefit, referred to as “medical prescription drugs”, when the prescription drug requires administration (or the FDA approved recommendation is administration) by a licensed healthcare provider (other than a pharmacist). Please note: Specialty prescription drugs meeting these requirements or recommendations are covered as a pharmacy benefit and not a medical benefit. These medical prescription drugs include, but are not limited to, medications administered by infusion, injection, or inhalation, as well as nasal, topical or transdermal administered medications. For some of these medical prescription drugs, the cost of the prescription drug is included in the allowance for the medical service being provided, and is not separately reimbursed.

  • Dental Care Plan The Welfare Plan will include a Dental Care Plan which will reimburse members for expenses incurred in respect of the coverages summarized in Appendix "1". The Plan will not duplicate benefits provided now or which may be provided in the future by any government program.

  • Health Care Benefits (a) Each regular full-time employee may elect coverage for himself and his eligible dependents* under one of the following health insurance plans:

  • Hospital and Medical Insurance The University shall make available health insurance to the employees covered by this agreement to the same extent and in the same manner as is available to other University employees, such as Faculty and the Executive, Administrative and Professional Staff employees. It is the University's goal to have the same health insurance plans offered uniformly to all University groups and employees.

  • Medical Exams 18.1: The Sheriff's Department may require a physical and/or psychological exam by a doctor, at the Employer's expense, to determine the employee's ability to perform his/her regular duties, if deemed appropriate. The employee may obtain a second opinion, at the employee's expense, and in the event there is a dispute between the Employer's doctor and the employee's doctor, both of these doctors shall select a third doctor, whose decision shall be final and binding on the parties. The expense for the third doctor's opinion shall be split 50-50 by the Employer and the employee if not covered by the employee's insurance.

  • Dental Care Benefits (a) The Employer shall provide such regular, full-time seniority employee (and his eligible dependents*) the 100/75/50 Co-Pay Dental Plan in effect as of the date of this Agreement, subject to such terms, conditions, exclusions, limitations, deductibles, co-payments and other provisions of the plan. The Employer shall pay 95% of the illustrated premium cost of such benefits and the employee shall pay the balance. Coverage shall commence on the day following the employee’s ninetieth (90th) day of continuous employment.

  • Extended Health Care Benefits 12.02(a) The City will provide for all employees by contract through an insurer selected by the City an Extended Health Care Plan which will provide extended health care benefits. The City shall pay one hundred per cent (100%) of the premiums, which will include any premiums payable under The Health Insurance Act, R.S.O. 1990, as amended. Eligible Expenses (Benefit year January 1 – December 31)

  • HEALTH AND INSURANCE BENEFITS 22.01 All health and insurance benefit premium costs paid by the Employer shall prorate in accordance with the proration formula under Article 22.12 of this Agreement. Same sex spouse is eligible to be a dependent for insured benefits.
