Life and Accidental Death and Dismemberment. The Board shall purchase through a carrier licensed by the State of Ohio, a $20,000 policy. The Board thereof shall pay the full cost of this coverage and increases. Every employee shall have the opportunity to purchase additional Life and Accident Death and Dismemberment coverage and other insurance coverage established by the carrier, within coverage and cost limits set by the carrier, through payroll deduction. Employees must enroll for additional coverage in writing, to the Treasurer during the established open enrollment period for additional coverage to be provided in the next contract year.
Life and Accidental Death and Dismemberment. The Employer shall maintain life insurance and accidental death and dismemberment policy coverage comparable to the policy in effect for employees at the time of ratification of this Agreement, subject to insurance company limitations. If the Employer determines that it is appropriate to change carriers or policies, it shall consult with the Union before undertaking any such changes.
Life and Accidental Death and Dismemberment. Each employee shall be provided with $50,000 (fifty thousand) life insurance and $50,000 (fifty thousand) accidental death and dismemberment insurance paid for by the City. Each employee shall have the option, at their own expense, to purchase additional amounts of life insurance and accidental death and dismemberment insurance to the extent provided by the City’s current providers. Evidence of insurability is contingent upon total participation in additional amounts.

In addition, the Board shall make available an unlimited amount at the employee’s own expense. (Individual rates may vary depending on the employee’s age).

Life and Accidental Death and Dismemberment. (AD&D) insurance: The City provides to each employee $50,000 in whole life insurance with ancillary AD&D coverage, at no cost to the employee. Employees may elect to purchase on their own additional life insurance coverages for themselves and their dependents. Coverage ends when employment or service with the City terminates.
Life and Accidental Death and Dismemberment. (AD&D) Reductions - Benefits reduce eight (8%) per year for principals actively at work, beginning at age sixty-five (65) and will reduce an additional 8% each year thereafter subject to a minimum benefit of $2000. Amounts of the insurance in excess of $200,000 are subject to evidence of insurability satisfactory to Underwriter.

Part-time Employees are ineligible for Life and Accidental Death and Dismemberment benefits. The Employee death benefit is equivalent to 100% of their annual salary to a maximum of $60,000, if the Employee meets the eligibility requirements. AD&D benefits are equal to the employee's life insurance benefits. As per IRS mandate, the School must include the premium cost of insurance over $50,000 on the Employees W-2.
