Linkages. The contractor shall have methods for coordinating care and creating linkages with external organizations, including but not limited to school districts, child protective service agencies, early intervention agencies, behavioral health, and developmental disabilities service organizations. At a minimum, linkages shall address: 1. Contractor's process for generating or receiving referrals, and sharing information; 2. Contractor's process for obtaining consent from enrollees or, where applicable, authorized persons to share individual beneficiary medical information; and 3. Ongoing coordination efforts (regularly scheduled meetings, newsletters, joint community based project).

Linkages. To further encourage use of Caltrain, bus and other transit services, and to enhance and encourage use of pedestrian and bicycle connections between the SUMC and downtown Palo Alto, the SUMC Parties shall fund the following improvements: (i) Improvements to Enhance Pedestrian and Bicycle Connection from Intermodal Transit Center to El Camino Real/Quarry Road Intersection. (ii) Public Right-of-Way Improvements to Enhance Pedestrian and Bicycle Connection on Quarry Road.
Linkages. In addition, there is a strong emphasis on linkage across the EDI programme, which is outlined in further detail in Annex B and should underpin the research conducted under this contract. Researchers are reminded that the terms of their award will detail the intellectual property of all outputs, giving DFID an irrevocable worldwide license to use all material produced through research, while allowing researchers to retain all intellectual property rights over that material. All outputs from EDI research will be global public goods. All primary data collection funded by EDI will be anonymized and made publicly available on a selected data repository in accordance with DFID’s Research Open and Enhanced Access Policy. All research teams will be responsible for cleaning data and preparing public use data. Data shall be publicly archived either 12 months after the end date of a research team’s EDI sub award; 24 months after completion of final data collection; or upon the publication of an academic paper in a peer review journal. This is outlined in more detail in Annex C.
Linkages. Contractor shall collaborate with regional community partners to ensure a continuum of supportive services to assist families in moving from supervised to unsupervised visitation. I. In consideration for services provided by the Contractor pursuant to this agreement, County shall pay the Contractor according to the payment schedule described below: A. County shall pay Contractor on a monthly basis for costs related to supervised visits, therapeutic visits, and supervised exchanges as described in Exhibit A, including all personnel expenses, operational costs, and direct program expenses. Said monthly payments shall not exceed $17,625 for the term of the contract. B. County may withhold all or part of Contractor’s total payment if the Director of Human Services Agency or her designee determines reasonably that Contractor has not satisfactorily performed the services described in Exhibit A. County will consider Contractor’s performance as being acceptable for the purposes of full payment of actual costs if Contractor meets each of the targeted outcomes as outlined in Exhibit A. 4.2, 4.3 and 4.4.
