Maximum Benefits. The maximum benefits We will pay are shown on the Schedule of Benefits of the Policy. We will not pay for Home and Community-Based Care on any day that You are confined in a Nursing Facility. Home and Community-Based Care benefits may be paid on any day You are confined in a Residential Care Facility provided the total benefits payable for that day will not exceed the Residential Care Facility Maximum Daily Benefit.

Maximum Benefits. The annual contribution for a Participant’s HSA Benefits is equal to the annual benefit amount elected by the Participant. In no event shall the amount elected exceed the lesser of: • The statutory maximum amount for HSA contributions applicable to the Participant’s coverage option (single or family) for the calendar year in which the contribution is made, or • The annual deductible under the HDHP option elected by the Participant. An additional catch-up contribution may be made for Participants who are age 55 or older. • The maximum contribution shall be reduced by any matching Employer contribution made on the Participant’s behalf • The maximum contribution shall be prorated for the number of months in which the Participant is an HSA eligible individual.

Maximum Benefits. The benefit year maximum is $1,000.00 for each benefit year for basic and major services. The lifetime maximum for orthodontic services is $1,250.00.

Maximum Benefits. The maximum aggregate benefits provided for each Subscriber per Calendar Year shall not exceed $1,000,000. The maximum aggregate benefits provided during each Subscriber's lifetime shall not exceed $2,000,000. This is the value of the benefits provided by Blue Shield to the Subscriber during his eligibility for Medicare without regard to any interruption in the continuity of Blue Shield membership.

Maximum Benefits. Benefits paid to any Covered Person for dental expenses for Preventive, Basic, and Major Services in any one calendar year shall not exceed the Maximum Benefit therefor as specified in the "SCHEDULE OF BENEFITS". Orthodontic Services are also subject to a lifetime Maximum Benefit as specified in the "SCHEDULE OF BENEFITS".

Maximum Benefits. Total additional increase for the final year of service of combined Basic and Other Benefits may not exceed 30% of the base contract salary for the final year of service. The base contract salary for an Administrator retiring during a contract year will be determined by prorating her/his salary over a full calendar year prior to the retirement date.

Maximum Benefits. The maximum benefits We will pay are shown on the Schedule of Benefits of the Policy. We will not pay for both Nursing Facility and Residential Care Facility benefits on the same day. We will pay other policy benefits while You are confined in a Residential Care Facility up to the Residential Care Facility Maximum Daily Benefit.

Maximum Benefits. (1) The total amount of benefits available during the lifetime of any one Participant under this Contract shall not exceed $5,000,000. (2) The maximum lifetime benefit amount includes all payments made under any benefit provision of this Contract for Network Benefits and Out-of-Network Benefits. (3) The maximum lifetime benefit amount is reduced in the amount of any benefits provided under the Subscriber’s Select 2000SM Plan Contract, the PPO SelectSM Plan Contract, and PPO Select AdvantageSM Plan Contract, PPO Select Saver SM Plan Contract, PPO Select ChoiceSM Plan Contract and PPO Select Blue Advantage SM Plan Contract held with Us immediately prior to a Participant’s Effective Date under this Contract. (4) All benefit payments made by Us for Physical Medicine Services, ground or air ambulance services, Extended Care Expense, preventive care, and prescription drugs, whether under the Network Benefits level or Out-of- Network Benefits level, will apply toward the Calendar Year benefit maximums under both levels of benefits.

Maximum Benefits. During their service with the Employer, employees will be eligible for benefits under the disability leave plan to the following maximum amount: 8-16 years 36 months 16 or more years 48 months All hours of paid disability leave benefits shall be counted toward the lifetime maximum limitation.
