Membership Application. Members or applicants for membership will complete and submit to AvMed such applications or other forms or statements as AvMed may reasonably request. If a Member or applicant fails to provide accurate information which AvMed deems material then, upon ten days written notice, AvMed may deny membership to such individual. Any person who knowingly and with intent to injure, defraud or deceive any insurer files a statement of Claim or an application containing any false, incomplete or misleading information is guilty of a felony, punishable as provided by Florida Statutes.

All members must have a current application and a signed Membership and Policy Agreement on file at the membership office of the Boys & Girls Club branch they attend • Check-in and check-out at the front desk is required each day of all members and non-members • Membership is for youth ages 6 – 18 only; if a young person is 18 years of age at time of enrollment, s/he must be enrolled in high school
Membership Application. Partner agrees that all the information provided on any registration or application form submitted to Siebel is, in all material respects, true and correct. Should there by any changes in such information during the course of this Agreement, Partner agrees to promptly inform Siebel in writing giving details of such changes.

During the life of this agreement the University’s Employment Services Manager, or her/his designee at the time of hire, rehire, reinstatement or transfer of an employee into the Bargaining Unit, shall apprise the prospective employee of this Article’s provision and shall present to her/him a Membership Application supplied by the Association. Membership Application. The Employer agrees to provide each new employee at the time of employment with a membership application, outlining to the employee their responsibilities in regard to Union membership and to provide the Union, in writing, with the name and address of each employee to whom the application was presented along with the employee's date of hire. The membership application shall be forwarded to the Union via scan and email not later than thirty (30) working days after the employee's date of hire. The Union shall bear the expense of printing the application form, the contents of the application to be such that it is acceptable to the Employer. The Employer further agrees to provide the Union, once a month, with a report containing the following information on a mutually agreed data processing medium: ⮚ Status PT or FT/ Temp ⮚ Current rate of pay ⮚ Name in full ⮚ Social Insurance Number ⮚ Mailing address ⮚ Email address ⮚ All known phone numbers ⮚ Termination date and reason for it ⮚ Amount of current weekly dues ⮚ Amount of current initiation fees ⮚ Amount of union dues assessment ⮚ Total dues for each employee for the current period ⮚ Total initiation fees for each employee for the current period ⮚ Start date The Employer agrees, upon written authorization from the employee, to deduct Union dues, applicant's service fee, initiation, and/or assessments, once each month and to transmit monies collected to the Union by the fifteenth (15th) of the following month, together with a list of the employees from whom such deductions were made.
Membership Application. Applicants shall:
(a) comply with the application process as defined and time by time updated on AFRINIC’s website (www.afrinic. net);
(b) provide accurate and complete information when applying for services. Incomplete application will not be accepted and applicant will be notified;
(c) clearly indicate the service(s) for which the application is being made;
(d) where the original information submitted has been the subject of any change, same has to be notified promptly, accurately and fully to AFRINIC by an authoritative and valid contact;
(e) promptly, accurately and completely reply to any enquiry made by AFRINIC during the application or within the currency of the agreement;
(f) provide the relevant information regarding the membership type as indicated online on AFRINIC website and reflected in the membership form;
(g) provide and ensure accurate contact information are stored in AFRINIC databases (Whois and MyAFRINIC).
Membership Application. We reserve the right to refuse membership during any time of the membership term and the member must abide by centre policies.
Membership Application. (a) A person (the applicant) who wants to become a member must apply to the corporation.
(b) The application must be in writing.
Membership Application. The Corporation will only accept a Membership Application submitted by the Employer on behalf of its Employees. The Corporation will not accept Membership Applications directly from Employees or Dependents.
