Membership Responsibilities. Adhere to governance structure within SELPA Local Plan and Policies, including designating appropriate representatives to serve on required councils and committees; ensure appropriate LEA representatives attend and participate in SELPA governance meetings as set forth in the Local Plan, Policies and Procedures.
Membership Responsibilities. The Employer agrees to provide each new employee, at the time of employment, with a form outlining to the employee their responsibility in regards to Union mem- bership, and to provide to the Union in writing, the name and address of each em- ployee to whom they have presented the form, along with the employee’s date of hire, the contents of the form to be such that it is acceptable to the parties. The Employer further agrees to provide the Union, once a month, with a list containing names of all employees who have terminated their employment during the previous month and a list of names of employees and addresses, as requested.
Membership Responsibilities. As a member, the Practitioner hereby agrees to observe and perform the following membership responsibilities:
a. Attend at least sixty ( 60) percent of the membership meetings per year.
b. Furnish, within the time period specified, responses to requests for information made by the Board, committee chairmen, officers of AcuPlan and consultants or advisors retained by AcuPlan.
c. Participate in programs and services offered to members by AcuPlan in accordance with the terms and conditions applicable thereto and otherwise support and further the goals, activities and purposes of AcuPlan.
d. Serve on the Board of Directors and/or such committees of AcuPlan as elected or appointed.
e. Participate as a provider in all health plans with which AcuPlan enters into provider agreements, either directly or on behalf of its members, pursuant to the terms of such provider agreements.
f. Discharge the responsibilities and duties accepted from or on behalf of AcuPlan in accordance with the best interests of AcuPlan.
Membership Responsibilities. Member agrees use of the Company premises, equipment, and classes shall be undertaken at his or her sole risk, and Company, its officers, employees, or agents shall not be liable for any harm, injuries or damage to Member or Member’s property, or be subject to any claim, demand, liability or damages whatsoever, including, without limitation, those resulting from acts of active or passive negligence on the part of Company, its officers, employees, or agents. Company shall not be responsible for any damaged, lost, or stolen personal property.

Membership Responsibilities. You acknowledge that Premier does not provide medical, clinical, diagnostic or therapeutic services but instead contracts with your Physician to do so. You and your insurance company will be responsible for paying any medical, clinical, diagnostic, or therapeutic services or items provided to you by WPA. The Program is not intended as a replacement of any health insurance or similar benefits program maintained by any Payor, and does not affect any applicable co-payments, co- insurance or deductibles thereunder (which you must continue to pay under the terms of such insurance or program). This Agreement is a service contract, and not a contract of insurance.
Membership Responsibilities. Members are fully responsible for the actions of themselves, their guests and family members. Members are fully responsible for their personal property and that of their guests and/or family members. In no case will TSS be held responsible for theft, loss or damage to any Member, guest or family property.
Membership Responsibilities. Scientists selected as affiliate members are expected to:
(1) demonstrate interest, willingness and availability to participate fully in the improvement of health conditions in low and middle income countries;
(2) contribute to scientific and technical exchange where appropriate;
(3) act according to ethical scientific standards;
(4) observe the current version of the CIHLMU statutes that are attached to this Membership Agreement;
(5) care about visibility of CIHLMU by mentioning the respective affiliation as well as by appropriate use of the logos of CIHLMU where applicable;
(6) inform the CIHLMU Office proactively about any changes in the particulars given in their application form;
(7) commit themselves to seek financial resources for joint projects that may be developed within the network, as membership is not related with any allocation of funds or resources.
Membership Responsibilities. You will (a) be responsible for You and Your Users' compliance with this Agreement, (b) be responsible for the accuracy, quality, and legality of Your Transactions, (c) use commercially reasonable efforts to prevent unauthorized access to or use of the NYSHEX Systems and Services and notify Us promptly of any such unauthorized access or use, (d) comply with the NYSHEX Member Handbook.
