By enrolling in the Changing Aging Program in the Community, I acknowledge that I am required to provide UBC BodyWorks with a medical Referral Form before I will be permitted to participate in the program. I also acknowledge that if my health changes during the term of my membership, I am required to inform UBC BodyWorks, and UBC BodyWorks may require me to obtain an updated medical Referral Form prior to continuing my participation in the program. If I am renewing my membership in the Changing Aging Program and my health has not changed, I will advise UBC BodyWorks and UBC BodyWorks may not require me to provide a new medical Referral Form, in its sole discretion. REFUND POLICY: I am aware that the UBC BodyWorks Outreach Programs does not grant refunds for any membership regardless of method of payment (unless: for medical reasons, in which case a note from a physician is needed; there is an extended work-related leave of absence accompanied with a note from your employer) and that all memberships are non-transferable. Refunds are only applicable to ACTIVE passes. Once passes expire, Bodyworks cannot refund purchases. HEALTH STATUS: I acknowledge and agree that it is my responsibility to ensure that I am physically and mentally fit to undertake the Changing Aging or Fit Over 50 Program in the Community offered by UBC BodyWorks, and that it is my responsibility to obtain medical clearance from my physician prior to my participation in the program if I am unsure about my ability to participate. Further, I acknowledge and agree that UBC BodyWorks may prevent or restrict my participation in the Changing Aging or Fit Over 50 Program if it believes that it is not safe for me to participate, in its sole discretion. These terms and conditions provide a criteria to the Board of Directors in considering applications for membership of Stockport Town Centre Business Improvement District (BID) Company Every BID Levy Payer is eligible to become a member of Totally Stockport, the Stockport Town Centre BID Company. Where a levy payer has multiple hereditaments, he/she is entitled to one membership/one vote. To become a member, a levy payer must complete the appropriate registration form. To become a member a levy payer/financial contributor must have paid their levy dues in full. No person shall be admitted as a member unless he/she supports the aims and objects of Totally Stockport the Stockport Town Centre Business Improvement District (BID) Company and wishes to take an active part in the support and furtherance thereof and his/her application for membership is approved by resolution of the Board. Members must work in the best interests of the company and its objectives and in supporting the agreed Business Plan, Rules set out in the BID Company Articles of Association and the Companies Xxx 0000 relating to the conduct of members/directors will also apply which include that lobbying other members or Levy Payers is not permitted. Registered members interested in furthering their involvement with Totally Stockport the Stockport Town Centre BID Company can formally apply to be a Board Director. This is subject to vacant seats and in accordance with the articles of association (retirement and rotation of directors). The aim in widening membership is to provide a broader representation of Levy Payers across all the sectors paying the levy within the BID area as part of Totally Stockport the Stockport Town Centre Business Improvement District (BID) Company. This will also help support the BID in its aim and provide members the opportunity to be involved in the company at the AGM and any other meetings specifically called by the Directors for Members. Members will not be able to stand for the position of Chair unless they have served a minimum of a year as a Board Director. Members must declare any conflicts of interest.
