Enrolment in Plans. Members and their dependants, if applicable, will be enrolled in the Extended Health and Dental plans unless they sign an exemption form. Members who have signed an exemption form may apply for coverage at a later date providing such coverage elsewhere has ceased. Application must be made within thirty-one (31) days of cessation of coverage. Insurability will be at the sole discretion of the carrier.
