PHLEBOTOMY SERVICES. Phlebotomy services, including blood culture draws, shall be available twenty-four (24) hours a day, seven (7) days a week. Eight (8) daily, phlebotomy rounds for all previously designated (excludes ED, all ICUs, PACU and OR) inpatient areas will be provided 7 days a week for all properly executed requests received in the Clinical Labs. (For electronic orders, cut- off is 5:30 am for 7 am draw, and 30 minutes previous to other draw times.) On-call phlebotomists will be available on each campus if a blood draw or culture is needed between scheduled rounds. All blood draws which cannot be obtained by the phlebotomy service shall be reattempted or reevaluated within 2 hours by the service, with the exception of overnight during the hours of 11 PM (2300) to 0400 AM. Due to the fact that there is only one phlebotomist scheduled during this time frame and per procedure, only 2 attempts are made per phlebotomist. In the event, the phlebotomist has to reschedule collection until 4 AM or unless the attending physician deems it clinically necessary for the House Officer to do the draw. All unsatisfactory or misplaced specimens obtained by the phlebotomy service will be redrawn by the service once notified by the provider with an updated request submitted in EMR. Phlebotomy will notify the patient’s nurse if he/she is ultimately unable to draw the blood, for whatever reason. Phlebotomy services will be available in the outpatient clinical areas during regular clinic hours, 7:30 am – 6:00 pm, with the exception of the Yawkey Ambulatory Care Center where the hours of operation are from 7:30 AM to 8:00 PM (2000).
PHLEBOTOMY SERVICES. The Hospital will maintain a program for the provision of phlebotomy services which will be primarily responsible for providing urgent and routine phlebotomy services. It is the Hospital’s goal that Residents will not be primarily responsible for providing routine phlebotomy services and that Residents normally will not be required to perform routine phlebotomy services unless it is required by their training program, as needed based on emergency patient care needs, or as necessary to develop the skill consistent with ACGME requirements. The topic of ancillary services shall be a standing agenda item on each labor management committee meeting. The Union will have the opportunity to communicate accurate information about ancillary issues to management at these meetings and the committee will attempt to formulate solutions.
PHLEBOTOMY SERVICES. Routine daily phlebotomy rounds in all inpatient areas will be provided seven days a week, beginning by 7 am for morning rounds and 2 pm for afternoon rounds, for all properly executed requests received prior to 6 am (afternoon: 1 pm).
PHLEBOTOMY SERVICES a. The Contractor shall provide a certified phlebotomist to perform Services at the Client Agency. The phlebotomist shall be responsible for drawing, preparing and at times providing transportation of samples (if courier is phlebotomist) directly to the Contractor’s laboratory for testing. b. The phlebotomist shall be available on weekends and State holidays at all times as needed or requested by Client Agency for routine and STAT (Immediate) Services. The Contractor shall maintain an on-call phlebotomist for STAT Services to accommodate any Client Agency inpatient needs. The phlebotomist shall provide the Client Agency an on-call phone number upon Contract commencement. If on-call phlebotomist telephone number changes, the State of Connecticut Contractor shall provide the Client Agency the updated telephone number prior to the updated telephone number going into effect. The phlebotomist shall arrive at the Client Agency within one (1) hour of receiving Client Agency’s phone call. c. The Contractor shall notify the Client Agency twenty-four (24) hours in advance of any modifications to the phlebotomist’s routine schedule via email and utilize the contingency plan for phlebotomist coverage so there is no lapse in scheduled Services. Contractor shall provide the Client Agency a copy of its contingency plan at the introduction meeting. d. The Client Agency may request a copy of phlebotomist certification at any time. The Contractor shall provide the Client Agency a copy of the certification within two (2) hours of receipt of the Client Agency’s email or phone call.
PHLEBOTOMY SERVICES. The Hospital will maintain a program for the provision of phlebotomy services which will be primarily responsible for providing urgent and routine phlebotomy services. It is the Hospital’s goal that Residents will not be primarily responsible for providing routine phlebotomy services, and that Residents normally will not be required to perform routine phlebotomy services between 6:00 a.m. and 11:00 a.m. However, it is recognized that proficiency in the performance of phlebotomy services is an objective of Pediatric Residency Training and Residents must perform this service as needed based on patient care needs or as necessary to develop the skill consistent with ACGME requirements. It is the responsibility of the Residents to work with the Hospital to identify deficiencies in ancillary services. This will include the provision of adequate information in order to assist the Hospital in identifying the reason(s) for these deficiencies and to enable the Hospital to take appropriate remedial action. The Parties agree to establish an Ancillary Services Committee for the purpose of discussing Resident’s performance of ancillary services. The Committee shall consist of the TPD and two additional members selected by the Hospital and three Residents selected by the Union. The Committee will discuss the performance of ancillary services with the goal of making recommendations for any proposed changes it can agree upon within one hundred and twenty (120) days of the effective date of this Agreement. The Committee shall have its first meeting within thirty (30) days of the effective date of this Agreement and shall meet at least once per month for four (4) months, at which point the Committee will be disbanded. The Hospital will consider any recommendations of the Committee for changes to the performance of ancillary services by Residents but will retain ultimate authority for determining how ancillary services are provided and who performs them, subject to the provisions of this Paragraph (T).
PHLEBOTOMY SERVICES. 5.1 The Contractor shall provide scheduled phlebotomy services to the Purchasing Agencies. Charges for scheduled phlebotomy services shall be included as a separate line item on the Contractor’s price list. 5.2 A Purchasing Agency currently receiving scheduled phlebotomy services, may instruct the Contracting Agency and Contractor to no longer provide scheduled phlebotomy services. The Contracting Agency and Contractor shall mutually agree upon either the revision of scheduled phlebotomy services to a Purchasing Agency, or the addition of a Purchasing Agency for scheduled phlebotomy services. 5.3 Purchasing Agencies, not receiving scheduled phlebotomy services may request phlebotomy services with seventy-two (72) or more hours advance notice to Contractor. Charges for seventy-two (72) or more hours advance notice phlebotomy services shall be included as a separate line item on the Contractor’s price list. 5.4 Purchasing Agencies not receiving scheduled phlebotomy services may request phlebotomy services with less than seventy-two (72) but more than two (2) hours advance notice to Contractor. Charges for phlebotomy services with less than seventy-two (72) but more than two (2) hours notice shall be included as a separate line item on the Conttractor’s price list. 5.4.1 As an alternative, phlebotomy services may be provided for Purchasing Agency patients at the Contractor’s specimen collection facilities. The Contractor shall provide a list of its specimen collection facilities to include current days/hours of operations. The charges for these specimen collection services shall be included as a separate line item on the Contractor’s price list. 5.4.2 Contractor shall develop procedures that Purchasing Agencies must follow when requesting the phlebotomy services described in 5.3 and 5.4. 5.4.3 Phlebotomists employed by the Contractor shall meet applicable certification and proficiency standards as required by applicable laws and regulations. 5.4.4 Contractor shall develop or have an active phlebotomy training/continuing education program for Contractor’s phlebotomists. Within thirty (30) days of a request by a Purchasing Agency, the Contractor’s phlebotomy plan/program design and confirmation that training has occurred for a specific phlebotomist, shall be available to the Purchasing Agency receiving phlebotomy services. 5.4.5 All medical waste generated by the Contractor’s phlebotomist shall be removed by the Contractor and disposed of in accordance with Fed...

  • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.

  • Telemedicine Services This plan covers clinically appropriate telemedicine services when the service is provided via remote access through an on-line service or other interactive audio and video telecommunications system in accordance with R.I. General Law § 27-81-1. Clinically appropriate telemedicine services may be obtained from a network or non- network provider, and from our designated telemedicine service provider. When you seek telemedicine services from our designated telemedicine service provider, the amount you pay is listed in the Summary of Medical Benefits. When you receive a covered healthcare service from a network or non-network provider via remote access, the amount you pay depends on the covered healthcare service you receive, as indicated in the Summary of Medical Benefits.   • Hospice Services Services are available for a Member whose Attending Physician has determined the Member's illness will result in a remaining life span of six months or less.

  • Anesthesia Services This plan covers general and local anesthesia services received from an anesthesiologist when the surgical procedure is a covered healthcare service. This plan covers office visits or office consultations with an anesthesiologist when provided prior to a scheduled covered surgical procedure.

  • Beta Services From time to time, We may invite You to try Beta Services at no charge. You may accept or decline any such trial in Your sole discretion. Beta Services will be clearly designated as beta, pilot, limited release, developer preview, non-production, evaluation or by a description of similar import. Beta Services are for evaluation purposes and not for production use, are not considered “Services” under this Agreement, are not supported, and may be subject to additional terms. Unless otherwise stated, any Beta Services trial period will expire upon the earlier of one year from the trial start date or the date that a version of the Beta Services becomes generally available. We may discontinue Beta Services at any time in Our sole discretion and may never make them generally available. We will have no liability for any harm or damage arising out of or in connection with a Beta Service.

  • Our Services As insurance intermediaries we generally act as the agent of our client. We are subject to the law of agency, which imposes various duties on us. However, in certain circumstances we may act for and owe duties of care to other parties, including the insurer. We will advise you when these circumstances occur, so you will be aware of any possible conflict of interest.
