Purpose of Request. FOR PROPOSAL (RFP) The State of Utah, Division of Purchasing & General Services (Lead State) is requesting proposals for office furniture and related accessories including all customer service, installation, and design services in furtherance of the NASPO ValuePoint Cooperative Purchasing Program. The purpose of this Request for Proposals (RFP) is to establish Master Agreements with qualified original equipment manufacturers (OEM’s) and their Authorized Dealers to provide office furniture products and related services for all Participating States. Authorized Dealers are required to offer customer service, sales service, warranty service, design services, and installation services. This solicitation does not allow for multiple firms represented by a distributor to respond. This solicitation does not allow for sister companies to team and submit offers. The objective of this RFP is to obtain best value, and in some cases achieve more favorable pricing, than is obtainable by an individual state or local government entity because of the collective volume of potential purchases by numerous state and local government entities. The Master Agreement(s) resulting from this procurement may be used by state governments (including departments, agencies, institutions), institutions of higher education, political subdivisions (i.e., colleges, school districts, counties, cities, etc.), the District of Columbia, territories of the United States, and other eligible entities subject to approval of the individual state procurement director and compliance with local statutory and regulatory provisions. The term of the Master Agreement shall be five (5) years. Any resulting Participating Addenda for a State of Utah Cooperative Contract will be for the benefit of all Utah public entities, nonprofit organizations, and agencies of the federal government, i.e. State of Utah departments, agencies, and institutions, political subdivisions (colleges, universities, school districts, special service districts, cities and counties, etc.). Purpose of Request. This Price agreement is for the lease of twelve (12) 4 passenger gas powered club cars. The agreement includes a maintenance plan at no additional cost to the City. Golf carts are in high demand at the Xxxx Xxxx Camp Store, expected to generate approximately $74,000 (52% profit margin) in rental revenue annually. To secure a 5% discount offer in addition to Sourcewell pricing, the Lake Parks division will initiate a contract with Metro Golf Cars with a start date of October 1, 2024 and will be subject to the FY25 approved budget. The annual agreement will replace the existing Golf Cart Lease Agreement with PNC Bank. The new agreement will increase the fleet on demand and will eliminate the need for rental units during peak season. Chapter 271.102 of the Local Government Code authorizes local governments to participate in a cooperative purchasing program with another local government or local cooperative organization. In lieu of competitive bidding, items and services may be purchased through such agreements as the agreements have already been bid by the sponsoring entity or agency. The City of Grand Prairie has master inter-local cooperative agreements with various entities including Sourcewell. Sourcewell can save money by pooling the impressive purchasing power of their members, which include hundreds of school districts, municipalities, counties, other local governments, and nonprofits across Texas. They use the power of numbers as leverage to get better prices with the same vendors we use now. Purpose of Request. This is an annual agreement for the purchase of rebar and concrete steel supplies to be used by the Public Works - Street Division for various projects throughout the city. Notice of bid #22124 was advertised in the Fort Worth Star Telegram and Planet Bids; it was distributed to 68 vendors in which 41 were HUB vendors. One bid was received as shown on attachment A. The low bid received from awarded vendor meets specifications and is recommended for award.
Purpose of Request. This first amendment to the Development Agreement with The Bakery Group LLC (Xxxx Baking Co. Building) would allow the City’s contractor to perform additional work under the City’s existing contract, funded by the original Development Agreement dated August 12, 2021. The Bakery Group LLC will pay the City $15,393 as part of this amendment, allowing the City to execute a change order with C&S Masonry to continue work along the Portage Avenue façade. Presently, pursuant to one or more stock option agreements, you have options ("Tyra Options") to purchase, at an exercise price of $1.00 per share, a total of ( ) shares of common stock of Tyra Technologies, Inc., a California corporation ("Tyra")—a subsidiary of Advanced Enterprise Solutions, Inc., a Delaware corporation ("Company"). To have MSC assume your options, a two-step process is required: first, immediately prior to the time the Merger is closed, we must exchange your Tyra Options for options to purchase Company's common shares ("Company Options"). Then, at the time of closing (the "Closing"), MSC will assume your Company Options. Purpose of Request. Data User has requested MHA to disclose to Data User data from a Data Set to be used by Data User consistent with this Agreement for the purpose(s) stated in an attached Addendum. Additional Addenda may be attached to this Agreement for multiple data requests by Data User. For any such request, Data User warrants that Data User's request is limited to the minimum necessary data to accomplish the intended purpose of the request and all further uses and disclosures of the Data Set by Data User must be necessary to carry out the stated purpose. Data User represents and warrants to MHA that the purpose(s) stated in the attached Addendum qualifies as research, public health or "health care operations" of a named "covered entity" as those terms are defined in HIPAA. Data User shall indemnify, defend and hold MHA harmless from all claims, liability, causes of action and damages (including civil monetary penalties or settlement amounts entered into with the Secretary of HHS or the Office of Civil Rights) arising out of or in any way relating to the purpose for which the Data Sets were disclosed to Data User or relating to MHA's reliance on the representations provided herein by Data User.
Purpose of Request. The original Development Agreement with South Bend Heritage Foundation (SBHF) regarding the Gemini at Colfax Apartments stipulated that $1M in City funding would be allocated to the property in exchange for a maximum for six (6) Permanent Supportive Housing vouchers being used at the property to help with the City’s housing issues. The original agreement had a completion date of December 31, 2021. The City successfully completed four (4) projects at the property: • Roof Replacement: $143,679 • Masonry Repairs: $368,128 • New Fire Stairs: $163,000 • New Water Line: $65,457 The City attempted a fifth project covering window replacement ($207,000), but the project stalled out during the COVID pandemic in 2021, leading to a contract cancellation through Board of Public Works in April 2022. SBHF maintains possession of the windows purchased by the City, and will work with a local contractor to install the remaining windows at their own labor cost. $70,000 remains from the original project budget after completion of the four projects mentioned above, as well as the contract cancellation regarding the windows. The City has been working with SBHF to scope out a small project to use the remaining funds. SBHF has a small masonry and sidewalk project scoped out to address deteriorating areas.
