Recipient Selection Criteria. ✓ Applicants will be selected on a first-come, first-qualified, first-served basis. ✓ Applicants must not own any other residential property at time of application and prior to closing for the past three (3) years. ✓ Applicants must receive a certificate of housing counseling upon completion of an eight (8) hour education and counseling workshop conducted by a HUD certified housing counselor. This certificate is required prior to loan application. ✓ All HOME proceeds must be used to pay closing costs; down payment; principal reduction and must NOT be used for debt consolidations, or cash-out to applicant. ✓ Lender must meet lender’s guidelines as established by County and/or City.
Recipient Selection Criteria. Applicants must be extremely low to low income households as defined by the SHIP Program. The Sponsor shall select eligible recipients based on their specified application process. The Sponsor will be required to select tenants on the basis of compliance as determined by the SHIP Special Needs Housing Program. The Inter-Local Agreement for the Special Needs Housing Program (Exhibit H) does not restrict special needs housing facilities as to geographical location; however the Sponsor’s service facility must be located within the incorporated areas of Alachua County.
Recipient Selection Criteria. Eligible applicants will be approved for assistance subject to funding availability using the following criteria: • Applicant referrals accepted from the local Continuum of Care (CoC) Coordinated Entry System/Homeless Management Information System (HMIS) to prioritize the provision of services. Exceptions may apply with City of Gainesville administrative approval. • Applicants must have a Vulnerability Index -Service Prioritization Decision Assistance Tool (VI-SPDAT) score of 5-10. The purpose of using the VI-SPDAT system is to quickly assess the health and social needs of eligible applicants and match them with the most appropriate support and housing interventions that are available within the community. Exceptions may apply with City of Gainesville/Alachua County administrative approval. • Applicants must meet income eligibility requirements. • Applicant may not be a college student unless employed 32 hours per week or more.

Recipient Selection Criteria. The Senior Transportation Services Program is implemented from the Cooper City Community Center. The Program is made available to all eligible seniors, 62 and older, who reside in Cooper City. Personal identification, to verify age and address of program participants, is kept on file. The Program is marketed through one or more of the following media outlets: news advertisement, City's website newsletter, social media sites, as well as on-site fliers. Specifically, seniors who are 62 years or older are considered limited clientele. Per CDBG federal regulations, seniors are presumed to be low-to-moderate income. The City of Cooper City maintains a file or registration form with each applicant’s identification which is stored in a confidential manner. The seniors register at the City's Recreation Adult Program Office located at the Cooper City Community Center. Files and records are available for monitoring by appropriate agencies/personnel upon request. Each participant will have an individual registration form and will be counted as unduplicated for registration purposes. Timesheets and a summary of expenses incurred through the Senior Transportation Services Program are maintained by City. These funds are documented in the Program's Monthly report and the portion of the Project that is to be reimbursed to the City through CDBG funding is documented in a monthly pay request.
