Applicant Eligibility. The CENTER shall determine and verify eligibility of applicants using a minimum of the following factors: A. The applicant is a resident of the City of Overland Park; B. The applicant’s household has an income at or below 30% (Very Low), 50% (Low), or 80% (Moderate) of the federal Average Median Income (AMI) levels; C. The applicant has a mortgage or rent payments in arrears; and D. The applicant has made a payment on their mortgage or rent within the past three months of the current bill. Notwithstanding the above, each household will be eligible to receive assistance only once per year.

Applicant Eligibility. The Transportation Department will review the driving history of all applicants for a school bus operator position (DHSMV Transcript of Driver Records) with the Florida Department of Highway Safety and Motor Vehicles (DHSMV). The Fleet Operations Coordinator will review the records twice per year as appropriate. Further, the Fleet Operations Coordinator reviews the Change Report from DHSMV every Wednesday. Determinations on applicant eligibility will be made in accordance with federal, state, and local laws, federal and state administrative code, and school board rules. The Transportation Department will screen each applicant's Transcript of Driver’s Record in accordance with the guidelines and criteria established by the document. The Transportation Department will review the entire driving record with particular emphasis placed on entries for five (5) years preceding the date of the records check. Although this plan is applicable to all current and prospective employees, the Transportation Department will not terminate employment of an employee based on their driving record prior to the original effective date of the plan. The Transportation Department will review the Federal Motor Carrier Safety Administration Drug and Alcohol Clearinghouse to determine eligibility for employment.

The applicant’s household has an income at or below 30% (Extremely Low), to 50% (Very Low), or to 80% (Low) of the federal Area Median Income (AMI) levels; C. The applicant has a utility xxxx in arrears, including services for electricity, natural gas, heating wood, propane, water, or sewage; and D. The applicant has made a payment on the utility xxxx within the past three months of the current utility xxxx. Notwithstanding the above, each household will be eligible to receive assistance only once per year.
Applicant Eligibility. A qualified applicant must be an attorney continually licensed to practice law and shall: • be a full-time employee of the state of Illinois or unit of local government (including tribal government) who prosecutes criminal or juvenile delinquency cases at the state or unit of local government level, including supervision, education, or training of other persons prosecuting such cases; or • be a full-time employee of the state of Illinois or unit of local government (including tribal government) who provides legal representation to indigent persons in criminal or juvenile delinquency cases including supervision, education, or training of other persons providing such representation; or • be a full-time employee of a nonprofit organization operating under a contract with Illinois or unit of local government who devotes substantially all of the employee’s full-time employment to providing legal representation to indigent persons in criminal or juvenile cases including supervision, education, or training of other persons providing such representation; or • be employed in Illinois as a full-time federal defender attorney in a defender organization pursuant to Subsection (g) of section 3006A of Title 18, United States Code that provides legal representation to indigent persons in criminal or juvenile delinquency cases. A qualified applicant must also: • have an outstanding balance on an eligible educational loan; • not be in default on repayment of any federal student loans; and • make a commitment to complete a term of service as described in the Service Agreement submitted with the application. Prosecutors who are employees of the federal government are not eligible. Attorneys who are in private practice and not a full- time employee of a non-profit organization, even if individually or part of a firm that is under contract with the state of Illinois or court appointed to provide public defense services, do not qualify as public defenders and are not considered to be qualified applicants.
Applicant Eligibility. UCS acknowledges, to the greatest extent possible, that CITY funds are intended to be used to benefit Overland Park households and families meeting Low- Moderate-Income criteria as established by the Department of Housing and Urban Development, and UCS will direct sub grantees accordingly. Applicant Eligibility. Any tribe or consortium of tribes seeking inclusion in the applicant pool must meet the following eligibility cri- teria: (a) Be a federally recognized tribe or a consortium of federally recognized tribes as defined in Public Law 93–638. (b) Document, with an official action of the tribal governing body, a formal request to enter negotiations with the Department of Interior (Department) under the Tribal Self-Governance Act authority. In the case of a consortium of tribes, the governing body of each participating tribe must authorize par- ticipation by an official action by the tribal governing body. (c) Demonstrate financial stability and financial management capability by furnishing organization-wide single audit reports as prescribed by Public Law 96–502, the Single Audit Act of 1984, for the previous three years. These audits must not contain mate- rial audit exceptions. In the case of tribal consortiums, each signatory to the agreement must meet this require- ment. Non-signatory tribes partici- pating in the consortium do not have to meet this requirement. (d) Successfully complete the plan- ning phase for self-governance. A final planning report must be submitted which demonstrates that the tribe has conducted— (1) Legal and budgetary research; and (2) Internal tribal government and organizational planning. (e) To be included in the applicant pool, tribes or tribal consortiums may submit their applications at any time. The application should state which year the tribe desires to enter negotia- tions.

Applicant Eligibility. State, regional, local and tribal governments, academia, non governmental organizations, utilities, and the private sector may apply for awards in this category. Existing statewide coordinating councils and/or other entities involved in the development of statewide geospatial strategic and business plans may apply for an award in this category. Coordinated multi-sector responses are strongly encouraged. Applicants from states that did not receive Fifty States Funding in Fiscal Year 2006 or 2007 will be given priority.
Applicant Eligibility. A. The application must be filed in partnership with the relevant Neighborhood Association(s). Only where no Neighborhood Association exists, applications may be accepted from a Non-Profit Organization, or other community stakeholder group that has proven to the satisfaction of the City Commission their intent to improve the conditions of the neighborhood. B. Any applicant and the partner organization must be in good standing with the City, including any contractual relationships and must not be in default to the City on any obligations. C. The applicants must be able to commit to actively engaging in a citizen participation framework to gather public input and negotiate in good faith. D. In order for the City Commission to consider an application for a VEDA for a specific project, the applicant (defined as the individual(s) and/or the entity which owns or proposes to own the property) must meet the following criteria: i. The applicant must have a legal interest in the property for which a VEDA is being sought, or must provide documentation evidencing its ability to acquire the property (i.e. binding purchase agreement). ii. The applicant must be compliant with the City’s income tax ordinance. iii. The applicant must not be delinquent on any financial obligations to the City iv. The applicant must not have any outstanding written orders or violations for any property under its control or ownership that is located in the City v. Before a VEDA which has been approved by the City Commission will be executed by the City, the applicant shall have filed a completed Michigan Department of Treasury Form 2766 – Property Transfer Affidavit L-4260 with the City Assessor related to its acquisition of the property. The Property Transfer Affidavit shall be considered incomplete if the purchase price of the real estate is not entered on the form. E. If the applicant’s project exceeds $600,000 or total employment upon completion of project is likely to equal or exceed fifteen (15) persons, then they must receive certification of equal opportunity practices from the City’s Office of Diversity and Inclusion according to the following guidelines. i. The applicant firm must complete and include all information requested on Forms EEO 200 (Covenant of Non-discrimination with the City of Grand Rapids), EEO 201 (Permanent Workforce Data), and EEO 202 (Covenant of Non-participation in Wage Theft Activities). Institutions that have received NSDI CAP Metadata Trainer and Outreach Assistance awards within the past three years are not eligible to receive a 2011 NSDI CAP award in this category.
