Rehabilitation Plan. If the chief executive officer makes the determinations necessary to provide rehabilitation services under this section and rehabilitation is agreed to by the parties, the retirement system shall designate a rehabilitation provider to evaluate the person and develop a rehabilitation plan.
Rehabilitation Plan. The Employer shall contribute two cents ($0.02) per hour worked to the BC Construction Industry Rehabilitation Plan in the manner set forth in Article 5.000.
Rehabilitation Plan. The parties agree to adopt the Preferred Schedule of Supplemental Contributions and agree to contribute to the National Industry Plan in accordance with any updated Preferred Schedule of Supplemental Contributions rate table that may be adopted.
Rehabilitation Plan. 36 The Crown adopted a Rehabilitation Plan con- sisting largely of steps to prevent siltation of water- courses, construction of erosion bars, clearing of leaning trees and pushovers, and sowing of grass seed from helicopters to establish vegetation to reduce erosion and improve slope stability. Natural regrowth of the forest will, of course, take decades. The elements and cost of this plan were agreed to by the parties.
Rehabilitation Plan. The parties agree to adopt the Preferred Schedule of Supplemental Contributions and agree to contribute to the National Industry Plan in accordance with any updated Preferred Scheduled of Supplemental Contributions rate table that may be adopted. For employees who have worked previously as a covered employee for another Employer contributing to the Service Employees Local 49 Pension Trust Fund, all service for the previous Employer will be counted for the purposes of the ninety (90) day exclusion. Contributions required by this provision will be paid to the Fund on or before the fifteenth (15th) day of the month following the period for which contributions are due or before such other date as the Trustees may hereafter determine. Contributions will be transmitted together with a remittance report containing such information, in such manner and on such form, as may be required by the Fund or their designee.
Rehabilitation Plan. The parties have adopted the Preferred Schedule of Supplemental Contributions and the Employer agrees to contribute to the SEIU National Industry Plan in accordance with the Preferred Schedule. The Employer further agrees to contribute to the National Industry Plan in accordance with any updated Preferred Schedule of Supplemental Contributions rate table that may be adopted by the Board of Trustees of the SEIU National Industry Pension Fund in the future.
Rehabilitation Plan. (a) The Care Planning Team is responsible for evaluating if a Client needs a Rehabilitation Plan. Rehabilitation Plans may be needed by Clients who receive therapy provided under direction of licensed therapists and the therapy plan requires regularly scheduled interventions by direct care staff such as but not limited to:
i. Speech therapy to assist Clients with speaking, reading, writing or swallowing;
ii. Bowel and bladder retraining;
iii. Activities to promote mobility (movement), muscle control, gait and balance;
iv. Exercise programs to improve movement, prevent or decrease weakness caused by lack of use, manage spasticity and pain, and maintain range of motion;
v. Nutritional counseling; and
vi. Activities to improve cognitive impairments, such as problems with concentration, attention, memory and poor judgment.
(b) Rehabilitation Plans must be developed and approved annually by a licensed therapist. The Rehabilitation Plan must define at a minimum Client specific interventions, the frequency of the intervention, the skill set needed to provide the interventions and outcomes that must be documented or reported to a nurse or licensed therapist. Rehabilitation Plans must be reviewed, documented and updated on a quarterly basis by the Care Planning Team.
(c) Contractor is responsible for coordinating or designating staff to provide regular communication with the licensed therapist on each Client’s response to the Rehabilitation Plan.
(d) Direct care staff shall receive training as needed to implement current Rehabilitation Plans.
(e) Staff designated to assist with Rehabilitation Plans shall document time spent on Rehabilitation Plan implementation in the Client’s file and have this documentation available to ODHS upon request.
Rehabilitation Plan. Lender’s approval of the preliminary plan for rehabilitation and capital improvements (including necessary expenditures for Code compliance) for the Facilities, based upon Lender’s inspection of the Facilities.
Rehabilitation Plan. The City shall prepare a rehabilitation plan for Rock Hill Landscape, using information from the research and written history under Stipulation I. The plan shall provide an accurate restoration planting plan for the gardens as well as recommendations for rehabilitating the garden terraces, stairs, walls, and other landscape features. The plans shall be prepared by a professional who meets the Secretary of Interior’s Professional Qualifications Standards for Historic Architecture (48 FR 44739) and has expertise in preservation landscape design. MACAA shall be consulted during development of the Rock Hill landscape plan.
Rehabilitation Plan. The Employer agrees to comply with the Rehabilitation Plan of the Automotive Industries Pension Plan (AIPP), effective 3/28/2008, pertaining to collective bargaining agreements renewed after 4/27/2008 in its entirety, including the payment of required supplemental contributions. For Example: Suppose an employer’s contribution rate to the AIPP is $100 per month for the life of a collective bargaining agreement expiring after 2014. The employer is obligated to pay effective 1/1/2013 supplemental contributions equal to 5%. Each year, the rate would increase 5% over the total effective rate of that previous year. In accordance with the 3/28/2008 Rehabilitation Plan, the total contribution rates to the AIPP would be as follows:
(1) Effective 1/1/2013, an amount of $105.00
(2) Effective 1/1/2014, an amount of $110.25
(3) Each year thereafter, the contribution rate to the AIPP will increase 5% over the previous year’s rate until the earlier of the expiration of the collective bargaining agreement or 12/31/2018. At contract commencement or renewal before 12/31/2018, the then-current Rehabilitation Plan of the AIPP would apply.
