Repayment Agreement. In consideration of the Administrator granting an Advance pursuant to the provisions of the Advanced Payments Program, the parties hereunto agree as follows:
Repayment Agreement. I HEREBY FURTHER AGREE to reimburse DHS, any and all hospital, medical and similar expenses paid on my behalf as a result of said accident or incident, should compensation be paid directly to me from any judgment, award, settlement, or insurance proceeds for my injuries received as a result of the above-referenced accident or incident. I FURTHER UNDERSTAND that this Repayment Agreement is in addition to any Assignment, right of recovery, right of subrogation, or lien rights DHS may have to any proceeds from any judgment, award, settlement, or insurance proceeds for my injuries received as a result of the above-referenced accident or incident.

Repayment Agreement. Your request to establish a repayment agreement has been: Accepted Denied Modified as follows: Your payment of $ must be received within the next 30 days of this notice and you must continue to send the monthly payment so that the payment reaches us by the 10th of each month or $ on and if the payment schedule is bi-weekly until your debt is paid in full. If your payment has not been received by the last day of the month, you will immediately become delinquent and your debt will be referred for collection. Your payment means that you accept the terms of this acknowledgment. If you fail to maintain the payments and do not renegotiate a new payment agreement, your debt will be delinquent and will be forwarded for collection. Collection activities are discussed below. If you have an overpayment that is not paid back, it will be referred for collection in a number of ways, including automated collection by the federal government. Federal benefits (such as Social Security) and tax refunds that you are entitled to receive may be taken to pay back the overpayment. The debt will also be subject to processing charges. This decision is based on 31 CFR 285. If you do not use your SNAP benefit account for a period of 365 consecutive days, any SNAP benefit remaining in the account that is at least 365 days old will be expunged (removed) from the account. Expunged SNAP benefits cannot be reissued. If you have a SNAP benefits overpayment, your expunged benefits will be put towards your overpayment. If you apply for SNAP benefits again, and have not repaid the amount you owe, your SNAP benefits will be reduced if you begin to get SNAP benefits again. You will be notified, at that time, of the amount of reduced benefits you will get.

Repayment Agreement. The Tenant shall be required to pay retroactive rent resulting from, Tenant’s underreporting or failure to report income, failure to provide adequate documentation, failure to report changes in a timely manner, or misrepresentation of income or family circumstances to the PHA. The PHA shall determine the retroactive rent amount as far back as the PHA is able to document. Upon determination of the retroactive rent amount, the family shall be required to enter into a Repayment Agreement with the PHA. The Repayment Agreement shall clearly state the total retroactive rent amount payable to the PHA in addition to the family’s regular monthly rent. The repayment time period in which the retroactive rent balance will be paid shall be based on the monthly payments determined and the retroactive balance. The PHA shall have full discretion in establishing the number of months in the Repayment Agreement for the family to pay the debt. A minimum rent hardship exemption previously granted to a family owing retroactive rent will be automatically revoked and the family will be required to pay the retroactive rent in full or enter into a Repayment Agreement for any retroactive rent owed, including rent owed during the hardship exemption period. Late and missed retroactive rent payments by the family shall constitute default of the Repayment Agreement. In such cases, the PHA shall have the right to terminate this Lease and obtain possession of the dwelling unit pursuant to available legal remedies.
Repayment Agreement. I hereby request the Department of Children and Families to deduct the amount agreed upon below, from my EBT account and further authorize them to deduct any future amounts from my account requested by me over the phone, by letter, or in person for repayment of existing overpayment debts.
Repayment Agreement. If your employment with Citi or one of its subsidiaries or their affiliates (jointly and severally “Citi”) terminates for any reason within 12 months after the start date of your Expatriate Assignment, other than if your employment is involuntarily terminated because of (a) a staff reduction or realignment of the work force causing elimination of your position, (b) a significant reduction, change or elimination of the need for specialized skills, (c) a relocation or dissolution of business, (d) the sale of any portion of a business if you are not offered a position by the acquiring organization at substantially the same salary rate, or (e) a schedule change that results in a loss of eligibility for benefits or a change in employment status from full-time to part-time (or vice-versa), you agree to repay the gross amount of all of the relocation expenses paid or reimbursed by Citi to you or on your behalf in accordance with the Citi Expatriate Program (your “Relocation Expenses”). For the avoidance of doubt, “gross amount” of your Relocation Expenses means the amount of each relocation benefit paid or reimbursed by Citi to you or on your behalf, plus the actual tax liability on such amounts paid or payable on your behalf by Citi pursuant to the tax equalization policy. You understand and voluntarily agree that the gross amount of any Relocation Expenses that you owe Citi may be deducted from any amounts or wages owed to you by Citi upon the termination of your employment, to the extent permitted by applicable law. If these deductions are insufficient to reimburse Citi fully, you will remain liable for the balance of such Relocation Expenses and agree to pay Citi that balance within three months after your termination date.
Repayment Agreement. As soon as practicable after the District notifies the State Treasurer that it does not expect to transfer a Debt Service Payment to its paying agent, or the paying agent notifies the State Treasurer that it has not received the Debt Service Payment by the required date or upon payment by the State Treasurer of a Debt Service Payment on the Bonds, the District and the State shall enter into a repayment agreement substantially in the form of the attached Exhibit A (the “Repayment Agreement”).
