(4) Within 30 days of sending the letter to the Student and the Parent, the District will provide OCR with a copy of any communications from the Student and the Parent in response to the District’s letter.
Reporting Steps. (1) The District will notify OCR within 10 days of the Student’s re- enrollment in the District.
(2) Within 30 days of the Student’s re-enrollment, the District will submit a report regarding its evaluation and compensatory education determinations, if the Student is eligible for special education transition services. The report will include a copy of the evaluation report, detailed meeting minutes, any remedial plans or revised IEPs developed for the Student, documentation of any input provided by the Student and/or the Parent, documentation that procedural safeguards were provided to the Student, and any other documentation relevant to the determinations reached by the group. If the District determines that no compensatory education is necessary, the District will provide OCR with a written explanation of the reasons for that determination, along with any supporting documentation.
(3) If applicable, every 30 days after the date the group first determines that the Student will receive compensatory education, the District will submit a report to OCR regarding the Student’s receipt of such education or services, including the type and amount of services provided, the dates the services were provided, and the name(s) and title(s) of the service provider(s). The final report is due 30 days after the last of these services have been provided.
Reporting Steps. (a) By August 26, 2022, the UW will submit to OCR for its review and approval a copy of its proposed letter to the Complainant. If OCR requires any changes to the proposed letter, the UW will re-submit them within seven days of receiving notice of the required changes. OCR and the UW will continue this process until OCR approves the proposed letter.
(b) By August 31, 2022, the UW will courtesy copy OCR on the letter to the Complainant.
Reporting Steps. (a) By August 26, 2022, the UW will submit to OCR for its review and approval its proposed plan as to which individuals will receive notice. If OCR requires any changes to the plan, the UW will re-submit the plan within seven days of receiving notice of the required changes. OCR and the UW will continue this process until OCR approves the plan.
(b) Within 15 days of OCR’s approval of the plan as to which individuals will receive the notice, the UW will submit a report demonstrating that the UW has disseminated the notice to all required individuals, including a copy of the notice and documentation that the notice was distributed.
(c) By September 30, 2022, the UW will submit to OCR for its review and approval its proposed plan as to which individuals will receive training. If OCR requires any changes to the plan, the UW will re- submit the plan within seven days of receiving notice of the required changes. OCR and the UW will continue this process until OCR approves the plan.
(d) Within 90 days of OCR’s approval of the plan as to which individuals will receive the training(s), the UW will provide a report to OCR demonstrating that it has completed its training activities. The report will include a copy of the training materials, and documentation of the dates and locations of the training sessions, and the individuals who attended the training sessions with their names and position titles.
Reporting Steps. (a) By July 13, 2018, or as soon as § II.D.4.a.(1) is completed, whichever is sooner, the District will provide OCR with a copy of its proposed letter. OCR will review it and provide feedback.
Reporting Steps. (a) By the effective date of this Agreement, the UW will submit to OCR for its review and approval a copy of its proposed guidance memorandum. If OCR requires any changes to the proposed guidance memorandum, the UW will resubmit the guidance memorandum with the required changes within seven days of receiving OCR’s notice of the required changes. OCR and the UW will continue this process until OCR approves the guidance memorandum.
(b) Within 15 days of OCR’s approval of the guidance memorandum, the UW will submit a report showing it has: (i) adopted and issued the guidance memorandum and communicated the relevant information in the memorandum to the UW community, including where and how Paratransit will pick up individuals with disabilities after games and the UW’s expectations that its game day partners honor their joint commitment to treat individuals with disabilities in a nondiscriminatory manner; (ii) implemented the memorandum for the 2022 football season; and (iii) updated the UW Husky Stadium annual operations plan, and UW Football’s website, written materials, and maps.
(c) For the 2022 and 2023 football seasons, every 45 days after the start of the season, the UW will submit to OCR a report summarizing whether the UW arranged to allow Paratransit to load and depart from the Paratransit pick-up location and if not, the reason(s) why; and whether the UW is aware of any reports or complaints that the pick-up and departure of Paratransit deviated from the UW’s guidance memorandum, and if it did, how the UW addressed, or plans to address, the deviation(s).
Reporting Steps a. By December 3, 2018, the college will submit to OCR, for its review and approval, a copy of the letter to the complainant. If OCR requires revisions to the letter, the college will re-submit the letter to OCR within 30 days of receiving notice of the revisions required by OCR. OCR and the college will follow the same process until OCR approves the college’s letter. Within 30 days of receiving OCR’s written approval of the letter, the college will provide OCR a copy of the final letter sent to the student.
Reporting Steps a. By December 3, 2018, the college will submit to OCR, for its review and approval, a copy of the procedures developed in accordance with Section II.A.1.a(1)-(3). If OCR requires changes to the procedures, the college will re-submit the procedures to OCR within 30 days of receiving notice of the revisions required by OCR. OCR and the college will follow the same process until OCR approves the procedures.
b. Within 75 days of receiving OCR’s written approval of the college’s procedures, the college will provide documentation to OCR demonstrating its compliance with Section II.A.1.b, including a copy of the final procedures and evidence of the adoption, distribution, and implementation of the procedures.
