SBBC Disclosure of Employee Records. (a) The employee records listed in this section are provided to INSTRUCTURE for the following purposes:
1) to provide support to SBBC users; and
2) for contract administration.
(b) SBBC will provide the following employee records to INSTRUCTURE through integration with the gradebook application:
1) first and last name 2) personnel identification number
SBBC Disclosure of Employee Records. (a) SBBC will provide vendor the records listed in 2.05 (b) for professional development services designed to create knowledge and understanding of Project Based Learning (“PBL”) and to support PBL implementation. This work will include an array of services and products, which are specified in the attached Statement of Work (“SOW”) (Appendix A).
(b) SBBC will provide BIE with the following employee records:
1) Name
2) Job Title
3) Work Location 4) Work Phone Number
5) Work E Mail Address
SBBC Disclosure of Employee Records. (a) SBBC will provide Conscious Discipline the records listed in this section for purposes of supporting teachers and administrators after training. Teachers and administrators may share their own names, email addresses, and phone numbers with trainers from Conscious Discipline or they may elect to have one point person at their school share information for all of the participating teachers.
(b) SBBC will provide Conscious Discipline with the following employee records:
(1) name (2) email address
(3) phone number
SBBC Disclosure of Employee Records. (a) SBBC will provide VENDOR with the employee records listed in this section for the following purposes:
1. For VENDOR to grant District employees with access to VENDOR’s systems, to ensure students are rostered correctly
2. For VENDOR to send reports on students’ test performance to teachers
3. For VENDOR to report scores back to teachers
(b) SBBC will provide VENDOR with the following employee records:
1) First and last name
2) Employee identification number
3) SBBC email address
4) Name of school or department
5) District identification number (District 6, Broward Schools)
(c) SBBC shall provide VENDOR with employee records through its integration program for the purposes listed in this section.
(d) VENDOR shall not use the employee records listed in this section for any purpose other than those listed above or re-disclose the records to any outside source without the prior written consent of the employee, except as required or allowable by law
(e) This section supersedes any of VENDOR’s privacy policies recollecting and using employee records.
SBBC Disclosure of Employee Records. (a) SBBC staff will provide BBHC with the employee records in this section for the following purposes:
1) to create an account for SBBC employees to access BBHC webportal to access information or make a student referral.
2) to notify account holder of students update
3) to determine supervisory roles
(b) SBBC will provide BBHC with the following employee records via secure email and or BBHC online portal.
1) SBBC employee (counselor, therapist or social worker) first and last name
2) SBBC employee’s email address
3) Employee’s direct supervisor
(c) The requirements of this section shall supersede any uses and disclosures of employee records or the like as listed in BBHC privacy policies, if any.
SBBC Disclosure of Employee Records. (a) SBBC will provide NAF the employee records listed in this section for the following purposes:
1) to create user accounts for staff to upload education records into the NAF data system
2) To advise SBBC staff on program coordination and components
3) To email employees program and other NAF opportunities and resources
(b) SBBC will provide NAF with the following education records via NAF’s data system:
1) Employee first and last name
2) Employee’s school email address 3) Name of school
SBBC Disclosure of Employee Records. (a) SBBC will provide VEI the employee records in this section for the following purposes:
1) to create teacher accounts for log in and use of VEI’s entrepreneurship program and global business simulation that facilitates the transformation of students into business professionals with an entrepreneurial mindset by bringing the workplace into the classroom.
2) to collect identifiable employee records (via teacher surveys) to improve the VEI program.
(b) SBBC will provide VEI with the following employee records via the SBBC’s integration system: Teacher first and last name.
(c) SBBC will provide VEI with the following employee records by inputting them into VEI system:
1) Username
2) Teacher’s first and last name 3) School name
SBBC Disclosure of Employee Records. (a) SBBC will provide employee records to LIA to coordinate the implementation of the program.
(b) SBBC will provide the following employee records to LIA:
1) Teacher name
2) Name of school
3) Work email address
SBBC Disclosure of Employee Records. (a) SBBC teachers complete their own application to receive BU training and a subsequent Montessori credential. SBBC teachers will provide their application directly to BU.
(b) SBBC will not disclose any additional employee records to BU without the prior written consent of the employee being trained, except as permitted or required by law.
SBBC Disclosure of Employee Records. (a) SBBC staff will provide VENDOR with the employee records in this section for the following purposes:
1) that meets Florida Sunshine State Standards.
2) In order to provide access to the online content available in our curriculum.
3) this Agreement.
(b) SBBC will provide VENDOR with the following employee records, via secure e-
1) Employee first and last name
2) Employee work email address
3) School location
(c) In addition to the employee records being disclosed by SBBC staff in this section, VENDOR will also have access to work and information input by employees in VENDOR's online platform, including, teacher generated customized lessons content and QuaverEd GradeBook/Rostering data.
(d) The requirements of this section shall supersede any uses and disclosures of employee records as listed in policies.
