SERVICE CREDIT RESTORATION PROCEDURE. A. Employees from other Xxxxxx Permanente Regions or former employees of Kaiser Foundation Health Plan of Colorado who are hired or rehired by the Employer in a temporary, on-call or short- hour status and then transfer into regular status must submit a written request to the Benefits Representative in the Human Resources Department to restore previous service.

  • Termination Procedure Regardless of basis, in the event of suspension or termination (in full or in part), the parties shall cooperate to ensure an orderly and efficient suspension or termination.   • Mediation Procedure The Chairman shall promptly advise the parties of a scheduled Mediation Hearing date.
