Service/Program Model. The Catholic Community Services of Western Washington Women’s Wellness Center is a health and hygiene facility for homeless women. The facility, located at 0000 0xx Xxxxxx in Seattle is open seven days a week. The hours of operation are 7:00 am to 4:00 pm Monday through Friday and 7:00 am to 2:00 pm Saturday, Sunday and holidays. Guests may rest while waiting their turn for hygiene services and while they are doing laundry. Services are provided at no cost to guests. The program provides a safe and peaceful environment where women can take care of their hygiene needs while being treated with dignity and respect. The Women’s Wellness Center partners with other programs and provides information and assistance to guest to help them access a range of services that include healthcare, housing, shelter, financial education, benefits assistance etc. Guest may also participate in one-on-one confidential consultation with a housing-focused case manager.

Service/Program Model. The St. Xxxxxx xx Xxxxxx Shelter Program located at 0000 Xxxxxxx Xxx Xxxxx provides shelter services for men over the age of 50. The hours of operation are 6:30 pm to 7:30 am. The program serves up to 212 men per night. During the winter season the program coordinates services with a network of churches to provide shelter to up to 34 additional men per night. In addition to night shelter, the program offers an evening meal, showers, toiletries, storage facilities, a clothing room, laundry facilities and a TV/reading room. A housing case manager assists people to access appropriate stable housing and a team of staff and volunteers xxxxxx client relationships that assist people with case management and information and referral services. St. Xxxxxx xx Xxxxxx Shelter Program partners with Lazarus Day Center to ensure continuity of services and support. Many of the program participants use services at Lazarus Day Center. Case Managers from both programs coordinate care for mutual clients. The program includes a respite service for up to 10 men nightly who may be referred by area medical facilities. A shelter nurse from Health Care for the Homeless provides nursing care for the men and monitors the respite program. Weekly Alcoholics Anonymous meetings are available on site and drug abuse counseling referrals are provided by staff. Twice per month an outreach worker form the Veteran’s Administration provides medical referrals and counseling to veterans in the program.
Service/Program Model. Lazarus Day Center is located at 000 0xx Xxxxxx Xxxxx in the Pioneer Square neighborhood of Seattle. The center is open daily from 1:00 pm to 6:30 pm and offers a safe environment for men and women 50 years of age and older who are vulnerable to climate conditions, street predators and illness. Services include computer access; a library and quiet room; lunch and snacks; a hygiene facility; a clothing room; laundry; case management and housing support; counseling information and financial services. The program serves people from a broad spectrum of cultures and experiences. Most clients have experienced chronic homelessness and other challenges that may include trauma, physical and mental disabilities, alcohol and drug dependency issues, physical or mental abuse etc. The program provides training and other tools to equip staff to respond effectively to clients. The Center offers the services of a Housing Case Manager to assist clients to address remove barriers and gain access to appropriate stable housing. Lazarus Day Center shares a number of mutual clients with St. Xxxxxx xx Xxxxxx Shelter. Staff of both programs work cooperatively to provide seamless effective services with the common goal of moving clients to stability. Lazarus Day Center expands site-based programming through collaboration with a number of social service and government agencies: Seattle University’s Nursing Program; Washington State Veterans Outreach Program; the U.S. Department of Veterans Affairs; and Healthcare for the Homeless. Such services may include healthcare, case management, education, benefits support and group work. In addition, staff coordinate housing services for clients who are disabled and 62 years of age and older with the Seattle Human Services Department Aging and Disability Division.

  • The Program The Program is a comprehensive commercial energy efficiency program that offers financial incentives and financing for qualifying energy efficiency measures in commercial buildings to customers who are property owners, tenants or managers (customers) of ACE in New Jersey. Customers must receive ACE electric delivery service and be in good standing. Incentives are available to customers for the purchase and installation of qualifying energy-efficiency measures at the location where the qualifying project is to be installed. XXX will not offer financial incentives for the same eligible measure to those customers who have received financial incentives or rebates from other ACE energy efficiency programs.

  • Managed Services HP will provide the services as described in a Statement of Work (“SOW”) attached to this Agreement or incorporating it by reference. Each party will appoint a single point of contact as set forth in the SOW who will serve as their primary representative, have overall responsibility for managing performance, and meet with the other party’s representative to review progress. Change requests are governed by the change management procedures as set forth in the SOW.

  • Third Party Service Providers Unless otherwise prohibited by Apple in the Documentation or this Agreement, You are permitted to employ or retain a third party (“Service Provider”) to assist You in using the Apple Software and Services provided pursuant to this Agreement, including, but not limited to, engaging any such Service Provider to maintain and administer Your Applications’ servers on Your behalf, provided that any such Service Provider’s use of the Apple Software and Services or any materials associated therewith is done solely on Your behalf and only in accordance with these terms. Notwithstanding the foregoing, You may not use a Service Provider to submit an Application to the App Store or use TestFlight on Your behalf. You agree to have a binding written agreement with Your Service Provider with terms at least as restrictive and protective of Apple as those set forth herein. Any actions undertaken by any such Service Provider in relation to Your Applications or use of the Apple Software or Apple Services and/or arising out of this Agreement shall be deemed to have been taken by You, and You (in addition to the Service Provider) shall be responsible to Apple for all such actions (or any inactions). In the event of any actions or inactions by the Service Provider that would constitute a violation of this Agreement or otherwise cause any harm, Apple reserves the right to require You to cease using such Service Provider.
