SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 5.1. The following is agreed in respect of the Services:
SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 4.1. The Fund appoints the Service Provider to define, develop and to submit to the Fund the Enterprise Architecture (EA) blueprint (‘Deliverables’) within a maximum period of 1 (one) year.
4.2. The Service Provider must, in defining and developing the Fund’s EA, comply with The Open Group Architecture Framework (TOGAF) and must have an understanding of the Government Wide Enterprise Architecture (GWEA) Framework.
4.3. The detailed specification of the Deliverables is more fully described in the Scope of Work contained in the Fund’s Request for Proposal,.
4.4. The Fund and the Service Provider shall, within one (1) month of the Effective Date, meet and agree on the final Project Implementation Plan, which shall specify, inter alia, the following:
4.4.1. Detailed description of the Deliverables and milestones;
4.4.2. Timelines within which the Deliverables and milestones must be provided;
4.4.3. Penalties; and
4.4.4. Billing milestones.
4.5. The agreed final Project Implementation Plan must be signed by the authorised representative of both Parties whereafter it shall be deemed incorporated into this Service Provider Agreement and binding. To avoid doubt, the Fund’s authorised representative for the purposes of this clause shall be Chief Information Officer.
SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 2.1. The Fund appoints the Service Provider to provide Claims Liability Valuation Software, End User Licenses and Support (hereinafter referred to as the “Services / Deliverables”) to the Fund as more fully set out in the attached Service Provider’s Proposal (Annexure B) and which service will include the following: Valuation/reserving methods supported by the software must include : o Stochastic techniques (bootstrapping and xxxx) o Development factor (chain ladder); o Xxxxxxxxxxx-Xxxxxxxx(expected loss); o Generalised Cape Cod; o Cash flow projection; o Average cost per claim and payments per claim finalised; o Settlement rate adjustment and case reserve adequacy adjustment (Xxxxxxxx & Xxxxxxx); and o Payments per claim incurred.
2.2. The parties are agreed that the Claims Liability Software will enable the following analysis: Estimating loss and claim reserves; Estimating Incurred But Not Reported (IBNR) liability; Evaluating loss development patterns; Estimating loss costs in reinsurance layers; Calculating averages and trends; Discounting loss and claim reserves; Estimating claim settlement/reporting patterns; Analysing loss adjustment expenses; Converting data into triangles; and Results selection and combination.
2.3. The details of the specific deliverables are set out in the Service Provider’s proposal documents and the Fund’s Request for Proposal Ref: RAF/2014/000...........
2.4. The Service Provider accepts the appointment on the terms and conditions set out in this Agreement.
SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 4.1. The Fund appoints the Service Provider to conduct an Organisational Structure Review and Skills Audit Assessment as a holistic service (‘Deliverables’).
4.2. The Deliverables shall include, but not limited to, the following:
4.2.1. Skills Audit;
4.2.2. Gap Analysis;
4.2.3. Organisational Structure Review; and
SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 4.1. The Fund appoints the Service Provider to erect Low Cost Infrastructure that is (i) speed humps, (ii) road signals, (iii) road signs, and (iv) guardrails etc. (‘Deliverables’) in the following province(s): 6
4.1.1. North West Province; and
SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 4.1. The Fund appoints the Service Provider to provide Coaching Programme for the Fund’s General Managers (‘Deliverables’).
4.2. The detailed specification of the Deliverables is more fully described in the Scope of Work contained in the Fund’s Request for Proposal annexed herein as Annexure B.
4.3. The Service Provider shall provide the Deliverables in accordance with the:
4.3.1. Scope of Work as set in Annexure B;
4.3.2. Service Provider’s Proposal annexed herein as Annexure C; and
4.3.3. Project Implementation Plan annexed herein as Annexure E, which will be annexed once completed by the Parties in terms of the provisions of clause 4.4 below.
4.4. The Fund and the Service Provider shall, within one (1) month of the Effective Date, meet and agree on the final Project Implementation Plan, which shall specify, inter alia, the following:
4.4.1. Detailed description of the Deliverables;
4.4.2. Timelines within which the Deliverables must be provided;
4.4.3. Billing milestones; and
4.4.4. Penalties.
4.5. The agreed final Project Implementation Plan must be signed by the authorised representative of both Parties whereafter it shall be deemed incorporated into this Service Provider Agreement and binding. To avoid doubt, the Fund’s authorised representative for the purposes of this clause shall be the Chief Executive Officer.
4.6. The Service Provider shall employ sufficient staff to ensure the Deliverables are provided as required under this Agreement.
4.6.1. The Service Provider must employ sufficient staff to ensure that the Deliverables are provided at all times and in all respects according to the Fund’s Request for Proposal reference RAF/2019/……………….., and any subsequent project implementation plan.
4.6.2. The Service Provider must ensure that a sufficient reserve of staff is available to provide the Deliverables to the Fund during absences of any staff.
4.7. The Service Provider must employ for the purposes of this Service Provider Agreement only such persons as are careful, skilled and experienced in the duties required of them, and must ensure that every such person is properly and sufficiently trained and instructed.
4.8. Where a Scope of Work provides that Deliverables shall be undertaken by Key Personnel, the Service Provider shall undertake all reasonable steps to ensure that the Key Personnel shall remain engaged for the full period of the Deliverable. SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 4.1. The Fund appoints the Service Provider to provide Courier Services for the distribution and delivery of documents, files and/or parcels of varying sizes for the Fund’s offices regionally, nationally and internationally:
4.1.1. Head office;
4.1.2. Regional offices;
4.1.3. Satellite offices;
4.1.4. Customer and Hospital Service Centres; and
4.1.5. Various other destinations (‘Deliverables’).
4.2. The Service Provider shall provide the Deliverables in accordance with the Scope of Requirements as set in Annexure B and shall include, but not limited to, the following:
4.2.1. Same Day Delivery Service;
4.2.2. Weekend Delivery Services;
4.2.3. After-hours Delivery Service;
4.2.4. Overnight Express;
4.2.5. Special Delivery Services;
4.2.6. Outlying/Remote Area Delivery;
4.2.7. International Delivery Service; and
SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 4.1. The Fund appoints the Service Provider to provide Statutory Actuarial Services (Deliverables) for a period of five (5) years.
4.2. The Deliverables are more fully detailed in the Scope of Work contained in the Fund’s Request for Bid and Annexure B and shall include:

The Fund appoints the Service Provider to provide efficient and effective telecommunication services including installation, maintenance and support (‘Deliverables’) to the Fund’s (i) Head Office, (ii) Regional Offices, (iii) Customer Service Centres and (iv) Hospital Service Centres.
4.2. The Deliverables shall include, but not limited to the following:
4.2.1. ISDN Primary Rate Interface (PRI) or Equivalent;
4.2.2. Public Switched Telephone Network (PSTN) or Analogue lines or Equivalent;
4.2.3. Diginet Lines or Equivalent;
4.2.4. Asymmetric Digital Subscriber Lines (ADSL) or Equivalent;
4.2.5. Very Small Aperture Terminal (VSAT) or Equivalent;
4.2.6. Virtual Private Network Supreme (VPNS) or Equivalent;
4.2.7. Share Call line. (Smart Access) or Equivalent;
4.2.8. 3G or Equivalent; and
4.2.9. Service Level Agreement (SLA) Requirements.
4.2.10. Ad-hoc Services as and when required.
4.3. The detailed specification of the Deliverables is more fully described in the Scope of Work contained in the Fund’s Request for Bid annexed herein as Annexure B.
4.4. The Fund and the Service Provider shall, within fourteen (14) days from the Effective Date, meet and agree on the Project Implementation Plan, which shall specify, inter alia, the following:
4.4.1. Detailed description of the Deliverables and milestones;
4.4.2. Timelines (days and dates) for project implementation;
4.4.3. Billing milestones;
4.4.4. Service Levels; and
4.4.5. Penalties.
4.5. The agreed Project Implementation Plan must be signed by the authorised representative of both Parties whereafter it shall be deemed incorporated into this Service Provider Agreement and binding. To avoid doubt, the Fund’s authorised representative for the purposes of this clause shall be the Chief Executive Officer.
4.6. The Fund may add additional sites as and when required for the provisioning of the Deliverables.
SERVICES, MATERIALS OR PRODUCTS SUPPLIED (DELIVERABLES. 4.1. The Fund appoints the Service Provider to provide experienced and suitable qualified resource(s), as and when required, to provide bid administration services to assist the Fund’s Supply Chain Management department in the acquiring of goods and services (‘Deliverables’).
4.2. The detailed specification of the Deliverables is more fully described in the Scope of Requirements contained in the Fund’s Request for Bid annexed herein as Annexure B.
4.3. The Fund shall, as and when required, during the subsistence of this Service Provider Agreement issue a detailed written Scope of Work Order for the required Deliverable(s) and which shall include, but not limited to, the following:
4.3.1. Detailed description of the Deliverables to be provided and time frames;
4.3.2. Number of resources required;
4.3.3. Time frame within which the resource(s) must be provided to the Fund;
4.3.4. Duration;
4.3.5. Billing Milestone; and
