The City of Los Angeles Department of Recreation and Parks reserves the right to alter/update restrictions and protocols, and/or cancel permits, as the County of Los Angeles Department of Public Health reopening protocols are updated and/or changed. Vaccination. The administration of an industry-recognized commercial vaccine by a registered licensed veterinarian, in accordance with the manufacturer’s recommendations, following a complete clinical examination, for prevention of disease.

Contractor acknowledges this Agreement may be terminated and payment withheld if this certification is inaccurate.
Vaccination a) In consultation with CTU, CPS shall coordinate with vaccination partners to prioritize CTU bargaining unit employees, according to the priority schedule below, for available vaccinations within phase 1(a) and (b). Additionally, Protect Chicago Plus is a community-focused vaccination initiative that targets and prioritizes 15 of Chicago’s most impacted and vulnerable communities. CPS employees who live in the designated communities or work in schools located in the designated communities will be prioritized in the initiative to the best of CPS’ ability. Through these partnerships, CPs shall offer vaccinations to clinicians, school clerks and clerk assistants before they are directed to work in-person in CPS buildings under this agreement. CPS shall make all concerted efforts to acquire at least 1,500 vaccine doses per week from vaccination partners.

Vaccination. The Company requires COVID-19 vaccinations as a condition of employment. If you have any questions about COVID-19 vaccinations, you should consult a trusted healthcare provider. Unless an exemption applies, the Company requires that all employees be fully vaccinated before their Start Date. An employee is considered “fully vaccinated” if at least 14 days or a period specified by the vaccine’s manufacturer have elapsed since the employee received the last dose of a vaccine that the FDA has authorized for use in the United States. FDA-authorized vaccinations include vaccinations that have been authorized pursuant to an Emergency Use Authorization.
